Objective:This clinical study observed the clinical efficacy of high intensity focused ultrasound combined with Chinese medicine in the treatment of adenomyosis and objectively evaluated the efficacy and safety of this treatment? Methods:A total of 99 patients who were clinically diagnosed as adenomyosis of the uterus and whose TCM syndrome was blood stasis syndrome and met the inclusion criteria and underwent high intensity focused ultrasound treatment in the affiliated hospital of chengdu university of traditional Chinese medicine from September 2017 to December 2019 were taken as the study subjects.According to the patients' wishes, the patients were divided into two groups: the experimental group(53 cases)continued to take traditional Chinese medicine within three months after the treatment, and the traditional Chinese medicine(TCM)was used as the second stage treatment;the control group(46 cases)did not take intervention measures. The changes of dysmenorrhea VAS score, menorrhea score chart,TCM syndrome score, days of dysmenorrhea, uterus volume,CA125,CA199 before and after treatment, and the volume of MRI focus before and after treatment were observed in all patientsResults:(1)Analgesic effect: the total effective rate of dysmenorrhea VAS score improvement test group was 83.01%,and that of control group was 78.26%,there was no significant difference(P > 0.05).The VAS score of dysmenorrhea decreased gradually at each time point of efficacy evaluation in the two groups. There was no significant difference in dysmenorrhea VAS score between the two groups in the first month after treatment(P > 0.05).There was significant difference in dysmenorrhea VAS score between the two groups in the third month after treatment(P < 0.05).There was no significant difference in the duration of dysmenorrhea between the two groups before and after treatment(P > 0.05).(2)The total effective rate of the experimental group and the control group was 86.11% and 62.50%,respectively(P < 0.05).The score of menorrhaphy in the two groups decreased gradually. The difference between the two groups was statistically significant(P < 0.05).(3)Curative effect of TCM syndrome: the total effective rate of TCM syndrome integral improvement test group was 71.70%,and the control group was 50.00%,the difference was statistically significant(P < 0.05).The TCM syndrome scores of the two groups at each evaluation time point decreased gradually. The difference between the two groups was statistically significant(P < 0.05).(4)Effect of reducing the volume of uterus and focus: the total effective rate of reducing the volume of uterus in the experimental group was 88.10%,and that in the control group was 75.00%,the difference was statistically significant(P < 0.05);after treatment, the difference between the two groups was statistically significant(P < 0.05).There was no significant difference between the two groups(P > 0.05).(5)Serum CA125 and CA199 levels: there was statistically significant difference between the two groups before and after treatment(P < 0.05);there was no statistically significant difference between the two groups before and after treatment(P > 0.05).There was no significant difference in serum CA199 between the two groups before and after treatment(P > 0.05)Conclusion:High intensity focused ultrasound combined with Chinese medicine two-stage treatment of adenomyosis has significant effect in reducing dysmenorrhea VAS score, reducing pain time, reducing menstrual volume, improving TCM syndrome and other aspects, reducing serum CA125 level and reducing uterine volume;the short-term effect of reducing focus volume is not ideal, and the improvement of serum CA199 level is not obvious. It is indicated that this method is effective in the treatment of adenomyosis.
