Clinical Observation Of HIFU Combined With Two-stage Therapy Of Traditional Chinese Medicine In The Treatment Of Adenomyosis

Objective:The purpose of this study was to observe the clinical efficacy of high intensity focused ultrasound(HIFU)combined with two-stage herapy of traditional Chinese medicine for treating adenomyosis(AM),thus evaluating the effectiveness and safety of this treatment.Methods:Forty-six patients who were diagnosed with adenomyosis and treated with HIFU at the Affiliated Hospital of Chengdu University of TCM were enrolled,including 26 patients in the experimental group and 20 patients in the control group.The experimental group was treated with HIFU combined with two-stage therapy of traditional Chinese medicine,and the control group was treated with HIFU alone.Uterine volume,clinical manifestations,TCM syndromes,improvement of quality of life,and changements of CA125 were compared between the two groups at 6 months after treatment.Result:1.After treatment,the total clinical effective rate of dysmenorrhea remission in the experimental group was 100.0%,and that in the control group was 80.0%,with statistically significant difference(P<0.05).The total clinical effective rate of menstrual volume decrease was 95.0% in the experimental group and 70.6% in the control group,and the difference was statistically significant(P<0.05).2.After treatment,uterine volume in both groups was smaller than before treatment,and the difference between before and after treatment was statistically significant(P<0.05).Uterine volume reduction rate of the experimental group was26.2%±16.8% and the control group was 14.0%±22.0%,the difference was statistically significant(P<0.05).3.After treatment,the total clinical effective rate of the remission of traditional Chinese medicine syndromes was 92.3% in the experimental group and 65.0% in the control group,and the difference was statistically significant(P<0.05).4.To both of groups,the difference of serum CA125 level between before and after treatment was statistically significant(P< 0.05),while the difference between the two groups was not statistically significant(P>0.05).5.To both of groups,the difference of quality of life scores between before and after treatment was statistically significant(P<0.05),while the difference between the two groups was not statistically significant(P>0.05).6.After treatment,the total treatment effect satisfaction rate of patients in the experimental group was 92.3%,and the control group was 65.0%,the difference was statistically significant(P<0.05).7.Dysmenorrhea score and menstrual volume score were positively correlated with uterine volume;serum CA125 level was positively correlated with dysmenorrhea score and uterine volume;quality of life score was positively correlated with dysmenorrhea score,menstrual volume score and TCM syndrome score.Conclusion:The intervention of traditional Chinese medicine after HIFU ablation treatment can improve the clinical efficacy of AM treatment,significantly alleviate systemic discomfort symptoms and improve the quality of life of patients.HIFU ablation combined with traditional Chinese medicine second-stage therapy is feasible and safe,with less adverse reactions,which provides a new and effective treatment for AM.
