Objectives To study the correlation between the simplified clinical pulmonary infection score(CPIS),pneumonia severity index score(PSI,fine risk stratification),inflammatory index [leukocyte count,neutrophil count,hyper sensitive c reactive protein(hs-CRP),procalcitonin(PCT)] and initial therapeutic efficacy judgment,clinical prognosis and complications in 138 patients with community acquired pneumonia(CAP),To explore the clinical value of CPIS ? PSI score and inflammatory index in evaluating CAP initial treatment effect and predicting the prognosis of patients with CAP.Methods Collect the sex,age,body temperature,pulse,blood pressure,respiratory frequency,oxygen index,consciousness,chest x-film,lung CT and the total number of neutrophils in blood routine leukocytes,PCT,hs-CRP,sputum culture results,arterial blood ph-value were collected.Blood urea nitrogen,blood sodium,blood glucose,sputum volume and sputum color,combined with basic diseases,and according to the relevant indicators to CPIS and PSI,PCT,hs-CRP.The simplified CPIS score(low,medium,high risk),PSI score(low,medium,high risk),blood routine leukocyte count,hs-CRP,PCT,were analyzed by means of ANOVA,chi-square test regression and multiple Logistic regression analysis.Results 1 Simplified CPIS score group(low,middle,high risk),PSI score group(low,middle,high),the level of inflammation index on day 1st was compared with the initial treatment effect,clinical outcome,and the occurrence of complications.Chi-square test was used,and the difference was statistically significant.2 The association between simplified CPIS group(low,medium,high risk),PSI score group(low,medium,high risk),blood routine leukocyte count at days 1st,3rd,and 7th and CAP initial treatment efficacy,outcome,and complications was analyzed by multiple logistic regression analysis.The simplified CPIS score group(medium,high risk)and PSI score group(medium,risk)were risk factors for poor clinical outcomes(30 days of death).The simplified CPIS score group(medium risk)and the PSI score group(medium risk)were all risk factors for complications.3 The simplified CPIS score group(low,medium,high risk)was compared with the CAP 1st,3rd,7th day inflammatory index.The variance analysis was used to CAP the inflammatory index(p<0.05).Difference was statistically significant among the groups with simplified CPIS score(low,medium,high risk).There was a positive correlation between the simplified CPIS score and the inflammatory index level at day 1st,3rd,7th using a linear regression analysis model.The inflammatory index level at day 1st was positively correlated with the simplified CPIS score(p<0.05).On day 3rd,the total number of blood routine leukocytes and PCT level were positively correlated with the simplified CPIS score.On day 7th,the total number of blood routine leukocytes was positively correlated with the simplified CPIS score on day 7th.4 PSI groups(low,medium,high risk)were compared with CAP inflammatory indicators.Analysis of variance was used.On days 1st,3rd,and 7th,CAP inflammatory indicators were statistically significant between low risk groups and middle risk groups,low risk groups and high risk groups(p<0.05).There was statistical significance between calcitonin level and PSI score group(low,middle,high risk)(p<0.05).The PSI score was compared with the CAP inflammatory index using linear regression analysis.The level of hs-CRP?PCT on day1 st was positively correlated with the PSI score(p<0.05).PCT level was positively correlated with PSI score on day 3rd,total number of blood routine leukocytes,PCT andPSI score on day 7th,hs-CRP and PSI score on day 7th.Conclusions 1 CPIS assessment sub-group(middle rist),PSI assessment sub-group(middle rist),day 1st hs-CRP levels are risk factors for ineffective initial treatment;simplified CPIS assessment sub-group(middle danger,high risk),day 3nd hs-CRP,day3 nd,day 7th hs-CRP are risk factors for CAP death,simplified CPIS score(middle rist),PSI score(middle rist),day 1st hs-CRP are risk factors for complications.2 The leukocyte count,hs-CRP,PCT level on day 3nd,the leukocyte count,hs-CRP level on day 7th are positively correlated with the simplified CPIS score,the hs-CRP,PCT level on day 1st,the total number of blood white blood cells,hs-CRP,PCT level on day 3nd,and the total number of blood white blood cells,hs-CRP,PCT on day 7th are positively correlated with the PSI score.Figure0;Table29;Reference 41...
