Objective : To Study biological indexes of procalcitonin(Procalcitonin,PCT),white blood cell count(White Blood Count,WBC),C reactive protein(C Reactive Protein,CRP)and community acquired pneumonia in community acquired pneumonia(community acquired pneumonia,CAP)the relationship between the severity,and the above three kinds of biology the diagnostic value of CAP index in judging whether patients need to go to the ICU treatment and prognosis.Methods: From September 2015 to September 2016 from Wangnan Medical College yijishan Hospital of 70 patients with CAP including respiratory medicine from 40 patients with CAP,ICU(ICU)from CAP in 30 cases of all patients diagnosed with "diagnostic criteria specified Chinese adult community acquired pneumonia diagnosis and treatment guidelines" in the 2016 edition.At the same time,SPSS19.0 statistical software was used to analyze the relationship between the biological indexes and clinical data.Results: The patients were divided into three groups according to CURB-65 score.The high-risk group of patients with length of stay(38 + 30 days)was significantly higher than that in the low risk group(13 + 10 days)in the risk group(14 + 8 days),the high-risk group of PCT(6.5 + 1.7),white blood cell count(12.8 + 5.2)higher than the number of white blood cell count in the risk group(11.5 + 2.9)PCT(5.5 + 0.2)and low risk group,white blood cell count(9.9 + 2.1)PCT(5.5 + 0.2),statistically significant(P<0.05)CD44 expression and age,gender,smoking history,histologicalgrade,TNM stage(P>0.05);the positive expression of CD44 with lymph node metastasis the rate was 75%(24/32),higher than those without lymph node metastasis47.2%(17/36),the difference was statistically significant;The level of PCT was significantly different between the three groups,the PCT level in the high-risk group was higher than that in the control group and the low risk group.According to Spearman correlation analysis the PCT level in patients with CRUB65 score was positively correlated with the severity(Spearman=0.788)and P was statistically significant than 0.05 ICU patients in the treatment group hospitalization was 42 + 32 day was higher than that in the non ICU group.White blood cell count of patients with ICU treatment group(12.5 + 5.2)and PCT(6.8 + 1.7)than non ICU treatment group of white blood cell count of patients with(10.9 + 3.2)and PCT(5 + 2),the difference between the two groups were statistically significant(P<0.05).PCT,WBC,CRP to determine whether the patients need treatment into the ICU area under the receiver operating characteristic curve(ROC-AUC)were 0.9580573 and 0.592 compared to the PCT area under the receiver operating characteristic curve is higher than that of CRP and WBC,showed that PCT in judging whether patients need to enter the ICU treatment has a higher diagnostic value.The death of patients age 76 + 19,PCT7.3 +2.1,white blood cell count 14 + 6.6(WBC)level is higher than the survival group age66 + 14,PCT5.4 + 1.9mg/L,11.2 + 3.5ug/ml and white blood cell count was statistically significant(P<0.05),the level of difference between survival group and death group CRP were no significant statistically meaning.PCT,WBC,CRP for the judgment of prognosis(death)treatment area under the receiver operating characteristic curve(ROC-AUC)were 0.861,0.584,0.693 compared to the PCT area under the receiver operating characteristic curve(ROC-AUC)was higher than that of CRP and WBC,showed that PCT in judging the prognosis of patients with higher diagnostic value.Conclusion: This study suggest that PCT,CRP,WBC in the diagnosis of the severity of CAP patients with diagnostic significance,of which PCT and CAP patients with the highest degree of correlation with significant diagnostic value.WBC is of higher diagnostic value in predicting whether the patient is entering ICU therapy and the prognosis of patients with PCT is higher than CRP.
