With the improvement of living standards,obesity and diabetes mellitus(DM),the incidence of metabolic diseases,such as rising trend year by year.Methylglyoxal(MG)as a highly reactive dicarbonyl compounds,are advanced glycosylation end products(AGEs)the main precursor,its formation and accumulation causes the increasing of the DNA and protein reconstruction,resulting in cell and tissue function disorders as well as aging and disease,such as obesity,DM,cancer,central nervous system diseases,etc.Elevated plasma MG level can lead to the occurrence of cardiovascular complications in DM patients,and is also an important risk signal for the occurrence and development of microvascular complications such as diabetic kidney disease(DKD).Obese plasma MG concentration can increase 2 times,Type 2 diabetes mellitus(T2DM)in patients with MG plasma concentration can increase 3-4 times,DKD group is even higher.At present,high performance liquid chromatography,gas chromatography-mass spectrometry,enzyme-linked immunosorbent assay(ELISA),capillary electrophoresis and electrochemical biosensor have been used to determine MG content in samples.Here,we will use ELISA to detect the plasma MG level in people with different metabolic disorders to explore the effect of MG on metabolism.Objective:This study observed the plasma concentrations of plasma MG in normal population,overweight and obesity,DM and DKD patients,compared the plasma MG levels in different groups,discussed the differences of MG levels in overweight and obesity,DM and DKD populations,and analyzed the possible influencing factors.Methods:1 Specimen collectionSelection of cangzhou city people's hospital outpatient and ward patients,107 cases of endocrine metabolism as the research object,divided into overweight anf obesity group(OAO,n=40),the DM group(n=44),DKD group(n=23),at the same time Cangzhou people's hospital physical examination center of physical examination for the normal control group(NC,n=33),the reasons for the experiment with the patient and sign a consent form.Venous blood was extracted from the subjects,and the plasma was retained and stored in the refrigerator at-40?C to measure the plasma MG level.2 Determination of clinical indicators(1)Age,height,weight and waist circumference were collected and body mass index(BMI)was calculated.(2)Records of patients with fasting insulin(FINS),fasting C peptide,glycosylated hemoglobin(HbA1c),total cholesterol(TC),triglyceride(TG),low density lipoprotein cholesterol(LDL-C),high density lipoprotein cholesterol(HDL-C),alanine aminotransferase(ALT),creatinine(Cr),urine and urine trace albumin creatinine ratio(ACR),by simplifying the MDRD formula glomerular filtration rate(eGFR).(3)plasma MG was determined by ELISA.Results:1.Comparison of clinical indicators between groups.There were no significant differences in age and gender composition between OAO group and NC group(P>0.05),and there were significant differences in BMI,waist circumference,visceral fat area,TC and TG(P<0.05).Compared with OAO group,there were no statistical differences in age,gender composition,BMI,waist circumference and Cr between the two groups(P>0.05),while there were statistically significant differences in FINS,fasting c-peptide,HbA1 c,TC and ldl-c(P<0.05).Compared with the DM group,there were no statistical differences in age,gender composition,BMI,FINS and lipid in the DKD group(P>0.05),while there were statistically significant differences inHbA1 c,Cr,ACR and eGFR(P<0.05).2.Comparison of MG level between groups: Plasma MG level in NC group was lower than that in OAO group,OAO group was lower than that in DM group,and DM group was lower than that in DKD group,with statistically significant differences(P<0.05).3.Pearson correlation analysis showed that plasma MG level was positively correlated with age,BMI,waist circumference,visceral fat area,HbA1 c,Cr and ACR(r values were 0.326,0.322,0.220,0.316,0.623,0.656 and 0.272,respectively,P<0.05),and negatively correlated with eGFR(r value-0.465,P<0.05).4.Multiple linear regression analysis of OAO group showed that visceral fat area and BMI were positively correlated with MG level,which were independent influencing factors of MG in OAO group(P<0.05).5.In the DM group,multiple linear regression analysis showed that HbA1 c was positively correlated with MG level and was an independent influencing factor of MG in the DM group(P<0.05).6.Multiple linear regression analysis of the DKD group showed that Cr and HbA1 c were positively correlated with MG level,while eGFR was negatively correlated with MG level,which were independent influencing factors of MG in the DKD group(P<0.05).Conclusion:1.Compared with healthy people,the plasma MG levels of overweight and obese patients,DM and DKD patients were all increased.2.MG level was positively correlated with age,BMI,waist circumference,visceral fat area,HbA1 c,Cr and ACR.EGFR is negatively correlated.3.The MG level of overweight and obese patients was related to the increase of visceral fat area and BMI;In DM patients,MG level was associated with HbA1 c elevation.MG levels in DKD patients were associated with elevated Cr,HbA1 c and decreased eGFR.
