Objective: To establish a risk scoring system of primary gastric cancer in Qingdao,the independent risk factors were screened and verified through statistical analysis of primary gastric cancer-related risk factors.It is of outstanding significance that healthy lifestyle of citizens in Qingdao was guided,and ability of recognizing primary gastric cancer in early stage for high-risk population was improved.What is the most monumental effort is that data supporting for the development of primary gastric cancer prevention strategies was provided,with a view to reduce the incidence and mortality of primary gastric cancer.Methods: A case-control study was designed.Age,gender,body mass index(BMI),history of smoking,history of drinking,history of Helicobacter pylori(HP)infection,history of chronic gastritis,history of diabetes,history of hypertension,history of coronary heart disease,family history of non-digestive system tumors,family history of digestive system tumors and income level were recorded in both gastric cancer group(N=500)and control group(N=500).SPSS18.0 and SAS9.2 software was performed for statistical analysis.Descriptive statistics were made on the general information of both gastric cancer group and control group.Chi-square test was analyzed to screen out suspicious risk factors that cause the occurrence of primary gastric cancer.Multivariate logistic regression analysis was used to analyze the interaction between multiple factors,when odds ratio(OR)>1 was indicated that the target factor was an independent risk factor for primary gastric cancer.All P values were 2-sided,and the significance level was set at 0.05.The preliminary risk scoring system for gastric cancer risk factors was established in Qingdao,Shandong Province,and the ROC curve was drawn for verification.Results: 1.Screening by univariate analysis: There was no statistical difference in data of age,gender,income level,history of diabetes,history of hypertension and history of coronary heart disease,respectively between two groups(P>0.05).There were statistically different in BMI,history of smoking,history of drinking,history of HP infection,history of chronic gastritis,family history of non-digestive system tumors and family history of digestive system tumors,what's more,these factors were classified as suspected risk factors;2.Multi-factor Logistic analysis verified: history of smoking(P=0.024,OR=1.473),history of HP infection(P=0.002,OR=5.954),history of chronic gastritis(P=0.033,OR=2.546),family history of non-digestive tumors(P=0.031,OR=2.514)and family history of digestive system tumors(P=0.004,OR=4.031)are independent risk factors for the occurrence of primary gastric cancer;3.A risk score system for primary gastric cancer in Qingdao was initially established.Through the verification of the ROC curve,it was found that the score system had a higher prediction accuracy for the incidence of gastric cancer.Conclusions: History of smoking,history of HP infection,history of chronic gastritis,family history of non-digestive system tumors and family history of digestive system tumors are independent risk factors for primary gastric cancer.It is shown that the occurrence of primary gastric cancer is the result of a combination of multiple risk factors.According to this,comprehensive prevention and treatment should be executed in order to prevent the occurrence of gastric cancer.Health literacy improvement projects for key populations should be comprehensively carried out by local medical institutions,in response to various types of gastric cancer risk factors.In order to achieve "early detection,early diagnosis,early treatment" diagnosis and treatment strategies,it's necessary to increase the coverage of early diagnosis programs in high-risk areas of gastric cancer,establish a model for identifying high-risk groups of regional gastric cancer,and improve the efficiency of early gastric cancer detection.Individuals should also raise their medical awareness on their own.Smoking should be quitted,alcohol intake reduced,physical exercise increased,eating habits controlled,and immunity strengthened.In addition,it's also requisite to take regular physical examinations and cancer screening.Once a stomach disease was found,treatment should be taken as soon as possible.For people with a family history of cancer,much more attention should be paid on their living habits and it's recommended that related tests should be conducted regularly.
