Objective: The objectivity of extracorporeal shock wave therapy treatment for cerebral palsy rehabilitation evaluation index research the therapeutic effect of triceps leg cramps,at the same time discuss extracorporeal shock wave treatment of cerebral palsy children the mechanism of action of triceps leg cramps.Methods: From December 2017 to November 2018,children with cerebral palsy and cerebral palsy with calf triceps spasm admitted to the rehabilitation department of the Department of Rehabilitation Treatment of Weihai City Hospital were selected as the research objects,strictly in accordance with the case admission criteria and exclusion criteria,Screening all children with cerebral palsy who intend to join the clinical study.A total of 19 children with cerebral palsy joined this clinical study,including 12 boys and 7girls Cases;the average age was 4.5 ± 1.8 years;9 cases of spastic hemiplegia and 10 cases of spastic quadriplegia.Random number table method was used to divide the enrolled children with cerebral palsy into extracorporeal shock wave treatment group(ESWT group)and control There were 9 cases in the group.Children with cerebral palsy in both groups received routine rehabilitation training as usual,including exercise therapy,physical factor therapy,sensory integration therapy,task-oriented training,and orthosis-assisted therapy.The ESWT group increases weekly extracorporeal shock wave therapy(treatment parameter settings: 15 mm diameter probe,pressure 16 bar,frequency 10 Hz,shock number 1500 times),and the control group increases weekly extracorporeal shock wave pseudo treatment(i.e.no therapeutic effect Effective extracorporeal shock wave therapy)The duration of treatment lasted 2 months;before and after treatment,the dorsiflexion angle(PROM),modified Ashworth scale(MAS),medial gastrocnemius muscle thickness(GMT),and modified Tardieu scale were evaluated(MTS),Heckmatt scale,Gross Motor Function Rating Scale(GMFM)and other indicators,record data in detailResults:1?Group compared:Compared with before treatment,children in the control group showed significant decreases in terms of dorsal flexion angle(t=6.996,P<0.05),MAS(t=4.000,P<0.05),MTS(R1: t=-6.733,P<0.05;R2: t=-3.345,P<0.05;R: t=-5.111,P<0.05),and significant increases in terms of GM thickness(t=-2.777,P<0.05)and GMFM score(t=-4.727,P<0.05).There was no significant change in Heckmatt score(t=-1.416,P>0.05).While children in the ESWT group showed significant decreases in terms of dorsal flexion angle(t=5.283,P<0.05),MAS(t=13.682,P<0.05),MTS(R1: t=11.090,P<0.05;R2: t=5.283,P<0.05;R: t=6.757,P<0.05),and significant increases in terms of GM thickness(t=-5.123,P<0.05)and GMFM score(t=-13.127,P<0.05).There was no significant change in Heckmatt score(t=0.092,P>0.05).2?Comparison between groups : Before treatment,therewere no significant differences between the two groups in terms of dorsal flexion angle(t=-0.037,P>0.05),MAS(t=-0.297,P>0.05),MTS(R1: t=-0.609,P>0.05;R2: t=-0.035,P>0.05;R: t=-0.722,P>0.05),GM thickness(t=-0.007,P>0.05),GMFM(t=0.118,P>0.05)as well as the Heckmatt score(t=0.092,P>0.05).After treatment,two groups of children with various index difference comparison before and after the treatment,there were significant differences in terms of dorsal flexion angle(t=-5.886,P<0.05),MAS(t=-3.356,P<0.05),MTS(R1: t=-5.326,P<0.05;R2: t=-2.451,P<0.05;R: t=-6.341,P<0.05),GM thickness(t=1.782,P<0.05),GMFM(t=0.452,P<0.05).There was no significant change in Heckmatt score difference comparison before and after the treatment(t=0.092,P>0.05).Conclusion:1?Extracorporeal shock wave therapy can effectively alleviate the calf triceps spasticity cerebral palsy,increase the patient's foot dorsiflexion Angle,children with lower leg triceps muscle tension,improve the level of gross motor function in children with.2?Extracorporeal shock wave therapy can increase children crus triceps muscle thickness,but not obvious effect on improving muscle fibrosis.3?It is speculated that extracorporeal shock wave therapy may play a leading role in relieving spasticity by influencing neurotic and non-neurotic factors.
