Objects:The purpose of this study is to explore the potential value of IVIM imaging for vertebral malignant lesions by analyzing the characteristics of IVIM imaging.Methods:In this study,19 patients with vertebral malignant lesions were included,including 8 cases of bone metastasis(29 vertebral lesions)and 11 cases of multiple myeloma(50 vertebral lesions).All patients completed Philips ingenia 3.0T MRI routine sequence and IVIM examination(b=0,20,40,80,200,400,600,800,1000s/mm2),and performed image post-processing to obtain D,D*and f values.All patients were divided into two groups according to the types of lesions.The differences of the above three indexes between the two groups were compared.The box chart was used to describe the distribution of each parameter value.ROC curve was used to analyze the differential value of the above three indexes for bone metastasis and multiple myeloma.Results:For malignant vertebral lesions,the values of D,D*and f were0.48(0.34,0.59)×10-3mm2/s,8.60±3.94×10-3mm2/s and 19.55(15.17,25.71)%,respectively.Further grouping study showed that the D and f values of bone metastasis group were 0.41(0.24,0.51)×10-3mm2/s,15.40(9.45,19.95)%,which were significantly lower than those of multiple myeloma group(0.52(0.41,0.70)×10-3mm2/s,22.33(16.50,26.96)%),and the difference was statistically significant(P<0.001).However,the D*values of the two groups were 9.16±4.35×10-3mm2/s and 8.27±3.68×10-3mm2/s respectively,with no significant difference(P>0.05).ROC curve analysis showed that the cutoff value of D value was0.45×10-3mm2/s,AUC was 0.742,sensitivity and specificity were 70%and 65.5%,respectively,and the cutoff value of f value was 19.72%,AUC was 0.762,sensitivity and specificity were 64%and 75.9%respectively,while D value had no value for differentiation of bone metastasis and multiple myeloma(P>0.05).Conclusions:IVIM imaging can quantitatively analyze the imaging characteristics of vertebral metastases and multiple myeloma lesions,and its D and f values have a certain differential value,while the application value of D*value is limited...
