Objective:To investigate the relationship between shear wave elastic modulus of breast lesions and the number of cells,blood vessel density,the content of collagen fiber in interstitial components.Methods:This was a retrospective cohort study of patients with breast lesions who underwent ultrasound-guided core needle biopsy,and routine ultrasound and shear wave elastography were performed before surgery.HE staining was performed on all patients'pathological specimens to make pathological diagnosis and count the number of cells.At the same time,the sections were subjected to VEGF immunohistochemical staining and Masson staining.Image-ProPlus6.0 image processing software was used to count the expression level of VEGF,quantitatively analyze the percentage of collagen fibers,observe the morphology of collagen fibers.Results:There were 42 breast lesions included in this study,of which 23 were malignant and 19 were benign.The maximum,mean,and minimum elastic modulus of breast malignant lesions were 143.56±69.41 kPa,119.97±66.25 kPa,84.75±58.51 kPa,and the ratio of the elastic modulus of the lesion to the surrounding tissue was 12.03±7.71.These values were significantly higher than those of benign lesions,and the value of benign were 50.1±26.91 kPa,38.18±22.53 kPa,25.81±18.85 kPa and 3.50±3.19(t=5.526,5.133,4.206,4.503,P<0.01).The area of collagen fibers in malignant lesions was 70114.69±13678.26?m~2,which was significantly larger than that of benign lesions47022.47±15051.32?m~2(t=5.156,P<0.01).The number of cells was 502.57±167.698,which was significantly smaller than that of benign lesions 838.11±211.886(t=5.371,P<0.01).The maximum elastic modulus was linearly positively correlated with collagen fiber content(r=0.646,P<0.01),and the maximum elastic modulus was linearly negatively correlated with cell number(r=-0.630,P<0.01).In 23 malignant cases,the positive expression rate of VEGF was 82.61%(19/23),and in 19 benign breast diseases,the positive expression rate of VEGF was 15.8%(3/19).The expression of VEGF protein in malignant breast lesions is significantly higher than that in benign breast lesions(P<0.05).Conclusions:Shear wave elastic modulus of breast lesions is closely related to the content of collagen fibers in tumors,and collagen fibers play an important role in the development of breast cancer as a cytoskeleton,which makes it easier to understand the value of stiffness in clinical applications.
