Objective: Cognitive dysfunction is one of the common comorbidities of epilepsy. Some patients with newly diagnosed epilepsy have cognitive impairment, but its mechanism and related risk factors are not clear. The purpose of this study was to assess the impact of clinical features, psychology, and social life on cognitive function in patients with newly diagnosed epilepsy. Methods: A total of 95 newly diagnosed patients with epilepsy were enrolled in the Department of Neurology,Shengjing Hospital,China Medical University from January 2018 to November 2019.In the same period,90 healthy volunteers who matched the gender, age and education level of the patients in the case group were included.Collect clinical data of patients and pass the Montreal Cognitive Assessment Scale(MOCA),Mini Mental State Scale(MMSE),Word Learning Test(AVLT),Digital Span Test(DST),Digital Elimination Test(DET),and Speech Fluency The sexual test(VFT)comprehensively assesses the level of cognitive function and impairment of the study subjects.Anxiety and depression scale(GAD-7),(PHQ-9)and quality of life questionnaire(SF-12)were used to evaluate their mental, psychological and social life.Newly diagnosed epilepsy patients were divided into normal cognitive function group and cognitive dysfunction group according to the MOCA scale score, and compared with univariate and logistic regression analysis to determine the independent risk factors of newly diagnosed epilepsy with cognitive impairment. Results:Compared with the healthy control group, the MOCA and MMSE scores of patients with epilepsy decreased significantly(P <0.05).Cognitive dysfunction of epilepsy is mainly manifested in severe impairment of computing power,short-term memory, attention, visual space and executive ability(P <0.001).Course,age of onset, education,VEEG epileptiform discharge, frequency of seizures, form of seizures, anxiety, depression, quality of life, and social activity are independent risk factors for cognitive impairment in newly diagnosed epilepsy patients(P <0.05).Conclusion:There is a decline in cognitive function in patients with newlydiagnosed epilepsy. There are many risk factors for comorbid cognitive impairment, and the cognitive field is extensively damaged.
