Effects Of The Heat-sensitive Moxibustion “Dubi” Acupoint On 5-HT,HA,SP And CGRP In Rabbit With Knee Osteoarthritis

Objective:To observe the effects of the heat-sensitive moxibustion "dubi" acupoint on the level of serum 5-hydroxytryptamine(5-HT),histamine(HA),substance P(SP),calcitonin gene related peptide(CGRP)in rabbits with knee osteoarthritis,and to provide some experimental basis for improving the mechanism of heat-sensitive moxibustion in the treatment of KOA.Methods:According to the random number table method,30 6-month-old male New Zealand rabbits with a body weight of 2.25-2.75 kg were randomly divided into blank group(n=6),model group(n=6)and moxibustion group(n=18).After 3 days of adaptive feeding,the rabbits in the blank group were fed normally without any intervention,but in the model group and the moxibustion group,0.5ml of 4% papain solution was injected into the knee cavity of the right lower limb through the patellar ligament of rabbits,the injection continued once every 3 days,three times in a row.At the same time,the rabbits in the model group and the moxibustion group were forced to move for 1 hour every day for a total of 15 days.When the molding was finished,the rabbits in the moxibustion group gently suspended the moxibustion at the right side of "dubi" point for 40 min,once a day,continued for a total of 7 days.Infrared thermal tomography imager and temperature recorder were used to measure the temperature of the abdomen,hip joint and ankle joint.When the temperature fluctuation of the rabbits within 7 days was all > 1?,they were divided into theheat-sensitive moxibustion group,with 10 rabbits.However,the temperature fluctuation of the rabbits within 7 days was all ?1?,they were divided into the non heat-sensitive moxibustion group,with 7rabbits.Six rabbits were randomly selected from each of the two groups.In the course of moxibustion process,the rabbits in the model group were fixed on the fixed platform,while the rabbits in the blank group were not allowed to intervene.During the experiment,the changes such as mental state,diet quantity,body weight and behavior of rabbits in each group were observed.On the end of the treatment,the levels of serum 5-HT,HA,SP and CGRP were detected by enzyme linked immune sorbent assay(ELISA).Result:1.Before modeling,the general condition such as mental state,diet quantity,body weight and Lequesne MG scores of the rabbits were not significantly abnormal in the blank group and the model group.After modeling,the general condition of the rabbits in the blank group showed no obvious abnormality,and their body weight showed a normal growth trend,in the meantime without change in Lequesne MG score.The rabbits in the model group showed pretty poor mental performance and a marked decrease in diet and water.When touching the limb injured with finger,their affected limb could be seen contracting,often accompanied by licking and sucking back.They tended to curl up in the corner of the cage and were particularly inactive.Even when walking,the lameness is obvious and the phenomenon of load-bearing on the healthy side appeared.It was visible that their hair all over the body is withered,yellow and dull,and the skin around the ears,eyes and lips is dark and pigmented.The body weight of the rabbits in the model group was lower than that before the modeling(P<0.05),and at the same time significantly lower than the blank group(P<0.01).The Lequesne MG score of the rabbits in the model group was significantly increased,higher than that before the model andin the blank group(P<0.01).2.After treatment,rabbits in the blank group were generally in good condition,while the general condition of the rabbits in the model group,the heat-sensitive moxibustion group and the non heat-sensitive moxibustion group was improved.Among then,the recovery of the rabbits in the model group was slower,and the improvement of the rabbits in the heat-sensitive moxibustion group was better than that in the non heat-sensitive moxibustion group.Compared with before treatment,the body weight of the rabbits in each group increased.Compared with the rabbits in the blank group,the body weight of the rabbits in the model group increased more slowly(P<0.01).The body weight of rabbits in the heat-sensitive moxibustion and the non heat-sensitive moxibustion group increased significantly,which was slightly lower than the blank group,but the difference was not statistically significant(all P >0.05).Compared with the rabbits in the model group,the body weight of the rabbits in the heat-sensitive moxibustion group and the non heat-sensitive moxibustion group showed significant increase(both P< 0.01),but there was no significant difference between the heat-sensitive moxibustion group and the non heat-sensitive moxibustion group(P>0.05).While the Lequesne MG score of the rabbits in the blank group showed no change,the Lequesne MG score of the rabbits in the model group,the heat-sensitive moxibustion group and the non heat-sensitive moxibustion group were all lower than before being treated,but still significantly higher than the rabbits in the blank group(all P <0.01).Compared with the rabbits in the model group,the Lequesne MG scores of the rabbits in the heat-sensitive moxibustion group and the non heat-sensitive moxibustion group were decreased(P<0.05,P<0.01),but there was no significant difference between the heat-sensitive moxibustion group and the non heat-sensitive moxibustion group(P>0.05).3.After treatment,the contents of serum 5-HT,HA,SP and CGRP of the rabbits in the model group were higher than those in the blank group(P<0.05,P<0.01).Compared with the rabbits in the model group,the contents of serum 5-HT,HA,SP and CGRP were decreased in the rabbits in the heat-sensitive moxibustion group and the non heat-sensitive moxibustion group(P<0.05,P<0.01),and the reduction of the rabbits in the heat-sensitive moxibustion group was better than that in the non heat-sensitive moxibustion group(P<0.05,P<0.01).Conclusion:1.The heat-sensitive moxibustion can improve the general condition,body weight and behavioral performance of KOA rabbit model.It shows the effectiveness of the heat-sensitive moxibustion in treating KOA.2.The heat-sensitive moxibustion can effectively reduce the level of serum 5-HT,HA,SP,CGRP in KOA rabbit model,and inhibit the inflammatory response,which is the possible mechanism of heat-sensitive moxibustion in the treatment of KOA.
