Objective: The purpose of this study is to study whether uric acid is related to the incidence of sudden deafness of different hearing threshold types,and whether it can be used as a pre-treatment observation index to predict the prognosis of patients with sudden deafness of different hearing threshold types,and how accurate the prognosis is.The correlation between mean platelet volume(MPV),neutrophil count-lymphocyte count ratio(NLR),fibrinogen and prognosis of patients with sudden deafness of different threshold types was analyzed.Methods: A total of 130 cases of patients with sudden deafness were diagnosed in the first place.They were grouped according to the type of audiogram(low-frequency group,high-frequency group,full-frequency group and all-deafness group).Another 60 healthy adults were selected as the control group to compare the levels of uric acid and other indicators in the peripheral blood of the two groups and the differences in uric acid and other indicators between the hearing curve groups.Each type was divided into an effective group and an ineffective group according to the prognosis.Age,disease duration,efficacy,uric acid level and related indicators were analyzed and compared between different groups.The effectiveness of each type was taken as the dependent variable.Univariate logistic regression analysis was performed first,then the independent variable P<0.1 in univariate analysis was further performed for multivariate logistic regression analysis.The area under the curve is calculated and the optimal truncation value is calculated.Result: The comparison between the sudden deafness group and the normal physical examinees showed that the blood uric acid level of the sudden deafness patients in the non-low-frequency group was higher than that in the normal physical examinees(t=2.685,P<0.05),indicating that high uric acid may be associated with the occurrence of sudden deafness,especially with the incidence of various types outside the non-low-frequency group,and it was the highest in the patients with high-frequency hearing loss.Logical regression results suggest that uric acid is a risk factor for non-low-frequency sudden deafness.(OR =1.009,95%ci: 1.005-1.014,P=0.000<0.05).Single factor regression analysis results show that the low-frequency outcomes in patients with sudden deafness and uric acid level,age,clinical time,fibrinogen and other related indicators(P < 0.1),and the prognosis of multiariable logistic regression analysis,and found that after adjustment for confounding factors on high levels of uric acid(a is 0 r = 1.008,95% CI: 1.002 ~ 1.015)may be low frequency group of the influence factors of the prognosis of patients with sudden deafness is good,uric acid value is higher,the better the prognosis.The area under the ROC curve of uric acid and good prognosis was 0.63,with progressive significance P=0.043,95%CI,[0.505-0.741].The optimal cut-off point for predicting good prognosis of patients with sudden deafness in the non-low-frequency group was 337.55umol/L,with a sensitivity of 55% and specificity of 74.6%.However,its predictive significance may need further testing and discussion.The remaining factors multiariable logistic regression analysis,we found that after removal of confounding factors interference,patients' age(r = 0.918,95% CI: 0.878 ~ 0.959),it's time to see a doctor(0 r = 0.926,95% CI: 0.878 ~ 0976),fibrinogen levels on admission to hospital(0 r = 0.504,95% CI: 0.256 ~ 0.990)is a low frequency independent risk factors for the prognosis of patients with sudden deafness.There was no correlation between mean platelet volume(MPV)and neutrophil count-lymphocyte count ratio(NLR).Conclusions: Relatively high uric acid is a risk factor for patients with sudden deafness,which is related to the incidence of high frequency,full frequency and total deafness group,and is mainly manifested in the influence on high frequency hearing area.Elevated uric acid is low frequency group of independent protective factors for a good prognosis in patients with sudden deafness,uric acid on the effect of sudden deafness in combination with uric acid physiological function(oxidation-oxidation paradox)were analyzed,and the elevated uric acid is in addition to the low frequency of occurrence,development of various types of sudden deafness risk factor,but the uric acid increased to a certain extent(ROC curve cutoff value 337.55 umol/L),uric acid excretion in the body acceleration,the extracellular antioxidant mechanism play a role,to protect patients with sudden deafness,and so on down uric acid treatment of sudden deafness needs further exploration.Uric acid has some value in predicting the prognosis of patients with sudden deafness of nonlow-frequency descending type,but its effect(AUC=0.63,close to 0.5)still needs to be further examined.In other aspects,the prognosis of sudden deafness was related to the patient's age,disease duration and fibrinogen content.The younger the patients with sudden deafness were(cut-off value was 40 years),the earlier they went to hospital(cutoff value was 5.5 days),and the lower the fibrinogen level(cut-off value was 2.38g/L),the better the prognosis was.
