Objective:To explore the ability of quantitative analysis of heart rate variability by Lorenz scatter plot,to obtain a better index of heart rate variability analysis to guide clinical work.Methods: From January 2015 to December 2018,345 patients were randomly selected for 24-hour Holter in our hospital.The standard deviation of 24-hour normal RR interval(SDNN)and the ratio of long axis(L),short axis(W),area(A)and long axis to short axis(L / W)of Lorenz scatter plot were collected.SDNN ≥ 100 was defined as normal HRV group,≥50,<100 as the mild reduction group and < 50 as the severe reduction group.Pearson correlation was used to analyze the correlation between the indexes of Lorenz scatter plot and SDNN.The range of Lorenz scatter plot indexes corresponding to normal,mild and severe decrease of HRV was calculated,and the prediction ability of Lorenz scatter plot indexes to the decrease and degree of HRV was tested by ROC curve.Result: 1.Compared with the normal HRV group,the SDNN,L,W,A,L / W in the mild reduction of HRV group and the severe reduction of HRV group were significantly reduced(P < 0.001).2.Pearson correlation indicated that L,W,A,L / W were positively correlated with SDNN(r = 0.903,0.593,0.827,0.299,P < 0.001).3.Single factor Logisticregression showed that L,W,A,L / W were the risk factors of HRV(or = 32.066,19.726,4.151,1.204,P < 0.05).4.The ROC curve showed that the AUC of Lorenz scatter plot was 0.965,0.833,0.929,0.579,P < 0.05,0.970,P < 0.001.The L was used as the diagnostic standard of HRV.When the best critical value(CP)was 3.575 cm,the sensitivity was 0.921,and the accuracy was 0.900.5.The AUC of HRV was 0.871,0.640,0.812,0.681,P < 0.001,and the combination of the four was 0.883,P < 0.001.When the CP is 2.475 cm,the sensitivity is 0.850 and the accuracy is 0.750.Conclusion: 1.There was significant correlation between the non-linear analysis index ——the L,W,A,L / W of Lorenz scatter plot and the linear analysis index SDNN.2.The reduction of L,W,A and L / W of Lorenz scatter plot can reflect the reduction of HRV,among which L has the greatest diagnostic significance,and the combination of multiple indicators can improve the diagnostic value of HRV reduction.3.When L is less than 3.575 cm,it can be diagnosed as slightly decreased HRV;when L is less than 2.475 cm,it can be diagnosed as severely decreased HRV.
