Objective:Proximal humeral fracture is a common shoulder joint injury.In many cases,the violence that causes proximal humeral fracture not only lead to fracture,but also involves soft tissues such as muscles and tendons around the shoulder joint.The most common one is the rotator cuff injury,which has no characteristic clinical manifestations when combined with proximal humeral fracture.Due to the lack of corresponding knowledge,ruptured rotator cuff may not be found by doctors during operation,thus consequently result in the dysfunction of shoulder.The repair of the rotator cuff injury after the fracture usually requires multiple surgeries,however the curative effect on patients may show a significant difference.The aim of this study is to explore the clinical efficacy of simultaneous and delayed surgery on repairing rotator cuff injuries of patients who suffered with proximal humeral fractures and full-thickness rotator cuff ruptures,by doing that we hope to provide data support for the treatment of such patients in the clinic.Methods: Retrospective analysis was conducted on 44 patients who suffered proximal humeral fractures associated with rotator cuff injuries during January 2015 and January 2017.24 of them had received the reduction and fixation of fracture and simultaneous treatment of rotator cuff injuries in our hospital,while other 20 patients only received a postponed repair of rotator cuff injury in our hospital(received the fracture reduction and fixation in outside hospitals).The shoulder joint function was evaluated by the Shoulder Scores(UCLASS)provided by the University of California,Los Angeles at 3 months,6 months and 1 year after surgery.Results: All the patients were followed up for 12?24 months.The UCLASS scores have shown that patients who received simultaneous surgical treatment possess much better results in the aspects of function,active flexion,flexion strength and subjective satisfaction than those who received postponed repair of rotator cuff injuries.The difference on the above aspects have statistically significant(P<0.05),while no statistically significant difference in the aspect of pain(P> 0.05).Conclusions: For patients who suffered proximal humeral fracture associated with full-thickness rotator cuff rupture,the clinical effect of simultaneous repair of rotator cuff injuries is better than that of delayed repair,the former could effectively improve the shoulder function of the patients.
