Objective With evidence as the means,theory as the framework,and practice as the basis,this paper preliminarily constructs an intervention protocol for the positive childbirth experience of primigravid women in natural birth based on self-efficacy theory,providing scientific psychological guidance for guiding primigravid women to actively cope with childbirth and maintain physical and mental health.Methods1.Literature analysis: systematically reviewed the literatures on the intervention of primigravid women' childbirth experience in the Chinese and English databases,and used content analysis method to extract the general information and the intervention situation of the included literatures,so as to form the first draft of the intervention protocol.2.Structured interview: structured interviews were conducted with parturients and obstetrics nurses in three third-class hospitals in Anhui province to understand stakeholders' opinions and suggestions on the first draft of the intervention protocol,and formed the second draft of the intervention protocol.3.Expert meeting: experts from different fields such as obstetrics and gynecology,psychology and maternal and child health were invited to discuss the second draft of the intervention protocol and initially form the intervention protocol.Results1.Literature analysis: after screening,26 literatures(23 studies)have been included.At present,most of the intervention studies aimed at the childbrth experience of primigravid women were carried out by midwives or other professionals.The intervention forms were face-to-face,one-to-one or group intervention,with the number of interventions ranging from 1 to 7.The intervention time was mostly in the late pregnancy or delivery,with the intervention time ranging from 30 minutes to 2 days.Based on the results of literature analysis,the first draft of the intervention protocol was formed: a randomized controlled trial design was adopted,and the intervention was carried out by the obstetric outpatient nurses.The intervention object were the primigravid women with natural childbirth intention;the intervention form was one-to-one + group,face-to-face intervention for six times,which was set at 28 W,30W,32 W,34W,36 W and 37 W,one-to-one intervention for 30 min and group intervention for60 min.2.Structured interview: 60 parturients and 40 nurses were interviewed The revised opinions on the intervention program were as follows: one-to-one intervention should last for at least 20 minutes,which should be extended according to personal situation and goal achievement,and group intervention for at least 30-60 minutes;in terms of intervention content,it emphasized that psychological intervention should be combined with health education,and pay attention to the role of social support,especially spouse support;other aspects included increasing the dynamic evaluation of maternity,providing comfortable environment,etc.3.Expert meeting: five experts in related fields were invited.The authority coefficient of the expert group was 0.895.The revision opinions on the intervention program were as follows: increased one online connection between the two interventions;increased the company of family members in the fourth intervention;the parturient who artificially asked for cesarean section during delivery should not be exclude,and need to interview to understand the reasons.Conclusions Based on the theory of self-efficacy and the statement of SPIRIT2013,through literature analysis,structured interview and expert meeting,this study preliminarily constructs an active childbirth experience intervention protocol for primigravid women in natural birth,which ensures the scientificity and feasibility of the intervention protocol in theory and practice,aiming to provide guidance for clinical nurses to carry out psychological intervention and ensure the health of mothers and infants.
