Research Purpose:In this study,through the analysis of the occurrence factors of intraoperative hypothermia and the changes of body temperature,two different thermal insulation nursing measures were implemented to protect the intraoperative heat preservation of elderly THA patients,to observe and analyze the clinical effect of compound thermal insulation method,and to further explore the best thermal insulation measures to prevent intraoperative hypothermia,so as to establish the standard of intraoperative body temperature protection and improve the quality of nursing in operating room.Research Methods:In this study,a randomized controlled trial was conducted with reference to the inclusion and exclusion criteria.94 patients who were scheduled to undergo total hip arthroplasty in the operating room of Huzhou Central Hospital from January 2019 to December 2019 were randomly divided into control group?n=47?and intervention group?n=47?.Perioperative compound heat preservation nursing measures?compound heat preservation method?were carried out in the intervention group.The body temperature,ASA grade,operation time,anesthesia time,PACU stay time,extubation time,postoperative hospital stay,thermal comfort score,bleeding volume,transfusion volume and washing volume were recorded when the patient entered the room?T1?,when the operation begins?T2?,the intraoperative 30min?T3?,the intraoperative 60min?T4?,the intraoperative 90min?T5?,the end of the operation?T6?,the time of entering the resuscitation room?T7?and the time of leaving the resuscitation room?T8?.Observing the postoperative shivering and postoperative complications.The collected data were sorted out and analyzed,and the data were processed by GraphPad Prism 8.0 and SPSS23.0 software.Research Results:1.In this study,a total of 94 elderly patients with THA who met the inclusion and exclusion criteria were collected,including 47 patients in the control group and 47 in the intervention group.There was no significant difference in the general data of the patients.The baseline levels of the two groups were the same and comparable.2.The body temperature of the two groups decreased gradually with the progress of operation,and there was no significant difference between the control group and the intervention group in body temperature?T1 and T2?,but during the period of?T3T8?,the body temperature of the control group decreased more significantly.The results showed that there were significant differences in body temperature between the two groups(Fgroup=37.365,P<0.001),time effect(Ftime=45.79,P<0.001)and interaction(F interaction=1.730,P=0.112).The results showed that there was significant difference in body temperature between the two groups.The results showed that there were significant differences in body temperature between the two groups,and the effects of time factors were not different in different groups.3.The incidence of hypothermia and shivering in the control group was significantly higher than that in the intervention group.The incidence of postoperative complications in the two groups was 27.7%and 23.4%respectively,and there was no significant difference between the two groups(c2=0.224,P=0.636).4.The postoperative extubation time,PACU stay time and postoperative hospital stay in the intervention group were significantly shorter than those in the control group.5.The score of thermal comfort in the intervention group?8.04±1.47?was significantly higher than that in the control group?6.49±1.04??t=-5.903,P<0.001?.Research Conclusion:1.Compound heat preservation method can maintain the stability of intraoperative body temperature and effectively reduce the incidence of intraoperative hypothermia in elderly patients undergoing total hip arthroplasty?THA?.2.Compound heat preservation method can shorten the time of tracheal extubation and recovery of anesthesia,reduce the incidence of postoperative complications and shivering,and improve the thermal comfort of patients.3.Compound heat preservation method can shorten postoperative hospital stay,increase humanistic care,promote patients'early and rapid recovery after operation,improve the quality of surgical nursing,and reduce the economic burden of patients.
