Objective :To analyze the relationship between vitamin D levels,thyroid autoantibodies,and the prevalence of thyroid nodules in middle-aged and elderly people with normal thyroid function in some areas of Gansu Province.Method:A total of 324 survey participants who participated in the National Survey of Thyroid Diseases and Iodine Nutrition Status-2014 in Gansu with normal thyroid function over 44 years old.They were divided into two groups based on the serum vitamin D levels of the respondents: the vitamin D deficiency group(serum 25(OH)D level ?20ng / mL)and the vitamin D relative sufficient group(serum 25(OH)D level >20ng / mL).The relationship between different vitamin D levels,Thyroid Peroxidase Antibody(TPOAb),Thyroglobulin Antibody(TGAb)levels,positive rates,and thyroid nodule prevalence were analyzed.Result:(1)The median vitamin D of the total survey population was 12.70(9.73,16.10)ng / mL,and the median vitamin D of men [13.96(10.85,17.86)ng / mL] was significantly higher than the median vitamin D of women [ 11.53(8.70,14.65)ng /mL,P <0.05].(2)The serum TGAb level in the vitamin D deficient group [10.52(10.00,20.16)IU / mL] was significantly higher than that in the vitamin D relatively sufficient group[10.00(10.00,11.71)IU / mL,P <0.05].(3)The prevalence of thyroid nodules(18.2%)in the vitamin D-deficient group was significantly higher than that in the relative adequate group(3.7%,P <0.05).(4)The levels of serum TPOAb and TGAb in male and female vitaminD-deficient groups were higher than those in the relative sufficient groups,but the difference was not statistically significant(P> 0.05).Serum TPOAb and TGAb levels in female vitamin D deficiency group [10.87(7.30,15.95)IU / mL and 11.42(10.00,33.20)IU / mL] were significantly higher than male vitamin D deficiency group [8.91(6.73,12.89)IU / mL and 10.00(10.00,15.26)IU / mL,P <0.05].(5)Spearman correlation analysis showed that female vitamin D levels were negatively correlated with TGAb(R =-0.257;P <0.05),and positively correlated with blood calcium and blood phosphorus(R = 0.196 and 0.191;P <0.05).(6)The positive rates of TPOAb antibodies,TGAb antibodies,and double antibodies in the male and female vitamin D-deficient groups were not statistically different from those in the adequate group(P> 0.05).The positive rate of TGAb antibody in female vitamin D deficiency group(22.7%)was significantly higher than that in male(7.5%,P <0.05).(7)The prevalence of thyroid nodules,the prevalence of single nodules,and the prevalence of multiple nodules in the male vitamin D deficiency group were higher than those in the relatively sufficient group,but there was no statistical difference(P>0.05);The prevalence of thyroid nodules(20.7%)and the prevalence of single nodules(15.3%)in women with vitamin D deficiency were significantly higher than those in the relatively adequate group [2.7% and 2.7%;P <0.05].(8)Using thyroid nodules as the dependent variable and the screened indicators as the independent variables,binary logistic regression analysis was performed.Both men and women showed a negative correlation between age and thyroid nodule disease(OR = 0.938,P = 0.003;OR = 0.934,P = 0.023).Conclusion:(1)The prevalence of vitamin D deficiency in middle-aged and elderly populations in parts of Gansu is relatively high,and attention should be paid to the vitamin D nutritional status of this population.(2)Vitamin D level is negatively correlated with TGAb.When vitamin D is deficient,women's TPOAb,TGAb levels and TGAb positive rates were higher than men's.(3)When vitamin D is deficient,the prevalence of thyroid nodules increases.Compared with the relative sufficient amount of vitamin D,the prevalence of thyroid nodules and the incidence of single thyroid nodules increased in women with vitamin D deficiency.
