Objective: To study the effects of different levels of ferritin and homocysteine on ventricular remodeling,the severity of cardiac dysfunction during hospitalization by observing the levels of FERR and HCY in elderly patients with heart failure with preserved ejection fraction,investigating the correlation between FERR,HCY with BNP,LAD,LVMI and LVEF,analyzing the relationship and the incidence between iron metabolism imbalance and hyperhomocysteine with the elderly HFpEF patients.Material and methods: Elderly HFpEF patients(n=71)hospitalized in the department of geriatric cardiovascular medicine of shaanxi provincial people's hospital from January to December 2019 were set as experimental group,healthy subjects(n=40)during the same period were set as normal control group.Collect General clinical data,biochemical test indicators,cardiac color doppler ultrasound parameters of all the selected subjects were collected and categorize for further use.Spss20.0 software package was used for analysis.Descriptive date was presented as the meanąstandard deviation for normally distributed variables and median for non-normally distributed variables.Independent sample T test was used for comparison between the two groups with normally distributed variables,and Mann-Whitney U test was used for comparison between descriptive data of non-normal distribution.Single?factor?analysis?of?variance was used to detected for the multiple testing of normally distribution variables,if not,Kruskal Wallis test was used.Categorical date were reported as per cent frequencies and compared by chi-square test.Pearson or Spearman correlation analysis was used to analyze the correlation of each variables.Risk factors of elderly HFpEF patients were analyzed using binary Logistic regression analysis.P < 0.05 is considered statistically significant.Results: 1.HCY?BNP?LVEF?LAD?LVMI in elderly HFpEF patients were higher than that in normal control group,and all differences were statistically significant(P < 0.05).2.In the elderly HFpEF group,HCY,BNP,LAD and LVMI were gradually increased with the increase of cardiac function classification,and the differences were statistically significant(P < 0.05),in addition to LVMI in cardiac function ? and cardiac function ?(P>0.05).The difference of FERR level between the two groups was not statistically significant(P>0.05).3.In the elderly HFpEF group,the incidence of iron metabolism imbalance was 47.9%.Including the incidence of iron deficiency was 15.5%,respectively,5.6%,2.8%,7.0% with the degree of heart failure,and the differences were not statistically significant(P > 0.05).Iron overload rate was 32.4%,respectively,4.2%,11.3%,16.9% with the degree of heart failure,and the differences were statistically significant(P < 0.05).4.In the elderly HFpEF group,there were some differences with the FERR level and the degree of heart failure.In the iron deficiency sub-group and the iron overload sub-group,the levels of BNP and LVMI were higher than those of the normal iron sub-group,and the differences were statistically significant(P < 0.05).5.In the iron deficiency sub-group and the iron normal sub-group of the elderly HFpEF group,there were a negative correlation between FERR and BNP,LVMI,but no correlation between FERR and LVEF or LAD.In the iron overload sub-group and the iron normal sub-group,there were a positive correlation between FERR and BNP,LVMI,but no correlation between FERR and LVEF or LAD.6.In the elderly HFpEF group,HHCY total incidence was 59.2%,5.6%,18.3%,35.2%,respectively with the degree of heart failure,the difference were statistically significant(P<0.05).7.In the elderly HFpEF group,HCY,BNP,LAD and LVMI in the HHCY sub-group were all higher than those in the normal HCY sub-group,and all differences were statistically significant(P < 0.05).8.In the elderly HFpEF group,there were a positive correlation between HCY and BNP,LAD,LVMI,but no correlation between HCY and LVEF.9.There was no correlation between HCY and FERR.10.Binary regression analysis showed that FERR and HCY were the risk factors of elderly HFpEF patients.Conclusions: 1.Serum ferritin and homocysteine levels were partly correlated with HFpEF in the elderly.2.Iron metabolic imbalance and hyperhomocysteinemia were risk factors of ejection fraction retention heart failure in elderly patients.3.The incidence of iron deficiency and iron overload in elderly HFpEF patients was high,and which was also associated with the severity of HFpEF cardiac dysfunction and ventricular remodeling in the elderly.4.The incidence of hyperhomocysteinemia was very high in elderly HFpEF patients and was related to the severity of HFpEF cardiac dysfunction and ventricular remodeling in the elderly.
