Object:1.To explore the effect of blood homocysteine level on the occurrence of arrhythmia in patients with heart failure with ejection fraction retention.2.Assess the risk factors of arrhythmia in patients with preserved ejection fraction heart failure.Method:Collected the patients with congestive heart failure who were hospitalized from September 2017 to December 2019 in Hulunbeier City People's Hospital,and divided them into two groups according to whether arrhythmia occurred.There were 100 patients with HFp EF(a group A)and HFp EF There were 100 patients without arrhythmia(group B),a total of 200 patients.Compare the general clinical data between the two groups,whether the blood homocysteine level,clinical blood related indicators,and echocardiographic related indicators were statistically different,and further use binary logistic regression analysis to evaluate the occurrence of ejection fraction retention heart failure patients The influencing factors of arrhythmia,for the arrhythmia group,use pearson analysis to explore the relationship between homocysteine and cardiogram related indexes,and explore its correlation with the occurrence of arrhythmia in patients with preserved ejection fraction heart failure.Result:1.Compare the general clinical data of the two groups: Group A age(68.1 ± 10.4)years> Group B age(60.2 ± 9.8)years,there is a statistical difference between the twogroups(P <0.05);80 patients with combination A and hypertension > 66 patients with combination B and hypertension,there was a statistical difference between the two groups(P <0.05);there was no statistical difference between the two groups in gender,ethnicity,smoking history,drinking history,diabetes history,coronary heart disease history(P> 0.05)).Comparison of clinical blood indexes between the two groups: blood homocysteine ??level in group A(16.9 ± 5.4)umol / L> blood homocysteine ??level in group B(12.9 ± 2.9)umol / L,there was a statistical difference between the two groups(P <0.05);NT-pro BNP(1384.2 ± 1048.7)pg / m L in group A> NT-pro BNP(724.4 ± 240.6)pg / m L in group B,the two groups were statistically significant(P <0.05);fasting in both groups There was no statistically significant difference in blood glucose,hemoglobin,white blood cells,blood lipids,liver function indexes and kidney function indexes(P> 0.05).Comparing the two groups of cardiac color Doppler ultrasound indexes: group A left and right anterior and posterior diameter(51.5 ± 3.4)mm> group B left and right anterior and posterior diameter 48.7 ± 3.1)mm,there was a statistical difference between the two groups(P<0.05);group A left ventricular diastole The anterior-posterior diameter of the end stage(54.2 ± 4.3)mm> the left and right ventricular end-diastolic diameter of group B was 50.3 ± 3.8)mm,the comparison between the two groups was statistically significant(P <0.05);the left ventricular mass index of group A(179.2 ± 16.6)g /m2> left ventricular mass index of group B(158.6 ± 16.6)g / m2,the difference between the two groups was statistically significant(P <0.05);left ventricular ejection fraction of group A(58.9 ± 6.8)% <left ventricular ejection of group B The score was(61.9 ± 5.7)%.There was a statistically significant difference between the two groups(P <0.05);there was no statistical difference between the left ventricular posterior wall thickness,ventricular septal thickness,and right and posterior anterior-posterior diameter of the two groups(P> 0.05).2.The above indicators that passed the 0.05 significance test were further subjected to logistic regression analysis of multiple risk factors.The results were that the age,Hcy,NT-pro BNP,LVEF,left and right anterior-posterior diameter,and left and right ventricular end-diastolic diameter passed the 0.05 significance test,And hypertension did not pass the significance test of 0.05.3.Pearson correlation analysis was performed.The blood Hcy level was positively correlated with the anterior-posterior diameter of the left atrium(mm)(r = 0.742,p =0.01),and the blood Hcy level was positively correlated with the anterior-posterior diameter of the left ventricle(mm)(r = 0.778).,P = 0.01),blood Hcy level was positively correlated with left ventricular mass index(g / m2)(r = 0.771,p = 0.01),blood Hcy was negatively correlated with left ventricular ejection fraction(%)(r =-0.653,P = 0.01),blood Hcy was positively correlated with NT-pro BNP(pg / m L)(r= 0.754,p = 0.01),and the above had statistical differences(P <0.05).Conclusion:1.The higher the blood homocysteine level,the greater the possibility of arrhythmia in HEp EF patients,and Hcy may cause arrhythmia by affecting the cardiac structure of patients with heart failure patients with preserved ejection fraction.2.Age,Hcy,left and right anterior-posterior diameter,left and right ventricular end-diastolic diameter,and left ventricular ejection fraction may be the influencing factors of arrhythmia in patients with preserved ejection fraction heart failure.
