Objective:To explore the clinical risk factors and morphological characteristics of ruptured anterior communicating aneurysms.Methods:A total of 149 patients with anterior communicating aneurysms who meet the inclusion criteria in our hospital were enrolled from January 2016 to June 2019.According to whether the aneurysms were ruptured,the patients were divided into two groups,the ruptured group(n=118)and unruptured group(n=31).The age,sex,smoking history,drinking history,hypertension history,aneurysm shape,daughter ascus,aneurysm length / neck width(Aspect Ratio,AR),aneurysm length / diameter of parent artery(Size Ratio,SR),multiple aneurysms,A1 segment variation and other factors of each patient were collected,and the single factor statistical analysis and multivariate logistic regression analysis were carried out.Results:1.The univariate analysis results of clinical characteristics showed that there was significant difference in smoking history between the ruptured group and the unruptured group(P < 0.05),but there was no significant difference in age,gender,drinking history,hypertension,diabetes and dyslipidemia(P > 0.05).2.Results of single factor analysis of morphological characteristics: there were significant differences between the ruptured group and the unruptured group(P < 0.05)in the shape of aneurysms,daughter ascus,aneurysm length,the AR value and the A1 segment variation(P < 0.05),but there were no significant differences in the aneurysms width,neck width,the SR value and the multiple aneurysms(P > 0.05).3.Multivariate logistic regression model was used to fit the variables that were statistically significant and possibly related to aneurysms.The results showed that: female(OR 8.579;95% CI: 1.218-60.416;P=0.031),smoking history(OR 14.507;95% CI: 2.161-97.393;P=0.006),daughter ascus(OR 13.917;95% CI: 1.676-115.542;P=0.015),high AR value(OR 10.561 95% CI: 2.468-45.193;P=0.001),A1 segment variation(OR 3.955;95% CI: 1.314-11.904;P=0.014)were independent risk factors for anterior communication aneurysm rupture.Conclusion:Women,smoking history,daughter ascus,high AR value and A1 segment variation are risk factors for anterior communication aneurysm rupture.
