The Expression And Significance Of IGF1R,SHC1,GRB2,MST1 And MDM2 In Seborrheic Keratosis,Bowen's Disease And Cutaneous Squamous Cell Carcinoma

Objective:In recent years,the incidence rate of skin tumors has been increasing year by year.It not only brings a serious threat to people's life and health,but also brings a heavy burden to people's economy.Therefore,it is essential to actively explore the pathogenesis of skin tumors and actively search for specific indicators in the pathogenesis of skin tumors.This study investigated the expression levels of IGF1 R,SHC1,GRB2,MST1 and MDM2 in seborrheic keratosis(SK),Bowen's disease(BD),cutaneous squamous cell carcinoma(cSCC),and analyzed their significance in order to provide a theoretical basis for the early diagnosis,differentiation or prognosis of skin tumors.Methods:1 The paraffin samples of 50 cases of SK,20 cases of BD,22 cases of cSCC and 10 cases of normal skin around pigmented nevus were collected from the Affiliated Hospital of Chengde Medical College and Cangzhou Central Hospital.The expressions of IGF1 R,SHC1,GRB2,MST1 and MDM2 in the above four groups were detected by immunohistochemical SP staining.2 Data were statistically analyzed by SPSS19.0 software,and the counting data were expressed as n(%).Pearson test,continuity correction test or Fisher's exact probability method were used for the comparison of positive rates,and the difference was statistically significant when P < 0.05.Results:1 The positive rates of IGF1 R in skin lesions of normal group,SK group,BD group and cSCC group were 0(0/10),64.00%(32/50),90.00%(18/20)and 90.91%(20/22)respectively.The positive rates in the cSCC group,BD group and SK group were higher than that in the normal group(all P < 0.05);the positive rates in the cSCC group and BD group were higher than that in the SK group(all P < 0.05);the positive rate in the cSCC group was not significantly different from that in the BD group(P > 0.05).(Tab.1)2 The positive rates of SHC1 in skin lesions of normal group,SK group,BD group and cSCC group were 0(0/10),88.00%(44/50),100.00%(20/20)and 95.45%(21/22)respectively.The positive rates of cSCC group,BD group and SK group were higher than that of normal group(all P < 0.05);there was no significant difference in the positive rates among cSCC group,BD group and SK group(all P > 0.05).(Tab.2)3 The positive rates of GRB2 in skin lesions of normal group,SK group,BD group and cSCC group were 0(0/10),18.00%(9/50),55.00%(11/20)and 72.73%(16/22)respectively.The positive rates in the cSCC group and the BD group were higher than that in the SK group and the normal group(P < 0.05);there was no significant difference in the positive rates between the cSCC group and the BD group,the SK group and the normal group(P > 0.05).(Tab.3)4 The positive rates of MST1 in skin lesions of normal group,SK group,BD group and cSCC group were 100.00%(10/10),80.00%(40/50),95.00%(19/20)and 72.73%(16/22)respectively,and there was no significant difference among the groups(P > 0.05).(Tab.4)5 The positive rates of MDM2 in skin lesions of normal group,SK group,BD group and cSCC group were 0(0/10),74.00%(37/50),100.00%(20/20)and 100.00%(22/22)respectively.The positive rates of cSCC group,BD group and SK group were higher than that of normal group(all P < 0.05);the positive rates of cSCC group and BD group were higher than that of SK group(all P < 0.05),there was no significant difference in the positive rates between cSCC group and BD group.(Tab.5)Conclusion:1 The positive expression rates of IGF1 R in the cSCC group,BD group and SK group were significantly higher than that in the normal group,and the difference was statistically significant.It was speculated that the up-regulated expression of IGF1 R might play a role in the development of the epidermal hyperplastic skin tumors.The positive expression rates of IGF1 R in the cSCC group and BD group were higher than that in the SK group,and the difference was statistically significant.It was speculated that IGF1 R might play an important role in the progression of the anaplasia and malignant transformation of keratinocytes in the epidermal proliferative diseases mentioned above for promoting and reference significance.The positive rate in the cSCC group is not significantly different from that in the BD group,it was speculated that IGF1 R may promote the occurrence of the above epidermal proliferative tumors,but has no predictive effect on the invasion and metastasis of cutaneous squamous cell carcinoma.2 The positive expression rates of SHC1 in cSCC group,BD group and SK group were significantly higher than that in normal group,the difference was statistically significant,indicating that SHC1 may be abnormally activated in seborrheic keratosis,squamous cell carcinoma in situ and invasive squamous cell carcinoma of skin.The positive expression rate of SHC1 was not significantly different among SK group,BD group and cSCC group,indicating that SHC1 may has a promoting effect on epidermal keratinocytes hyperplasia,but has no obvious promoting effect on its anaplasia change,malignant change,invasion and metastasis.3 The positive expression rates of GRB2 in the cSCC group and the BD group were significantly higher than that in the SK group and the normal group,and the difference was statistically significant,but the difference between the SK group and the normal group was not statistically significant,suggesting that GRB2 may be involved in the occurrence of cutaneous squamous cell carcinoma,while its role in seborrheic keratosis,a benign tumor,was not obvious.The positive rate in the cSCC group was not significantly different from that in the BD group.These results suggest that GRB2 may not be able to predict the invasion and metastasis of cutaneous squamous cell carcinoma.4 The positive expression rates of MST1 were not significantly different among normal group,SK group,BD group and cSCC group in multiple comparisons,suggesting that MST1 may not be involved in the occurrence and development of seborrheic keratosis and cutaneous squamous cell carcinoma.5 The positive expression rates of MDM2 in the cSCC group,BD group and SK group were higher than that in the normal group,and the difference was statistically significant,suggesting that the up-regulation of MDM2 expression may play a promoting role in the occurrence and development of the epidermal proliferative tumors mentioned above.The positive rates in the cSCC group and the BD group were higher than that in the SK group,and the difference was statistically significant,suggesting that MDM2 might play an important role in the progression of the anaplasia and malignant transformation of keratinocytes in the epidermal proliferative diseases mentioned above for promoting and reference significance.There was no significant difference of positive rate between the cSCC group and the BD group.It was speculated that MDM2 might not be able to predict the malignant degree of cutaneous squamous cell carcinoma.
