Objective:To observe the clinical effect of laparoscopic surgery and open surgery in the treatment of cervical cancer.Methods:We performed a study involving women who underwent radical hysterectomy for stage ?A2 or ?B1 cervical cancer during the January 2012 to June 2016 period at our hospital,Retrospective analysis of clincical data of 130 cervical cancer patients.According to the different operation methods,they were divided into the open surgery group(control group)of 68 cases and the laparoscopic surgery group(observation group)of 62 cases.The operative time,intraoperative bleeding,postoperative exhaust time,postoperative catheter retention time,the length of hospitalization,postoperative complications and other surgical indicators were compared between the two groups.After 3 years follow-up,the recurrence rate was compared between the two groups.Results:The average age of the patients in the laparoscopic surgery group was 50.03 ± 10.20 years old,The average BMI of the patients in the laparoscopic surgery group was 22.80±2.03kg/m2.According to the FIGO stage,22 patients were stage ?A2 cervical cancer,40 patients were stage ?B1 cervical cancer;according to the postoperative pathology,50 patients were squamous cell carcinoma and 12 patients were adenocarcinoma.According to the family history,there were 3 patients with family history and 59 patients without family history.The average age of the patients in the open surgery group was 48.43 ± 8.99 years old,The average BMI of the patients in the open surgery group was 23.34±2.42kg/m2.According to the FIGO stage,21 patients were stage ?A2 cervical cancer,47 patients were stage ?B1 cervical cancer;according to the postoperative pathology,58 patients were squamous cell carcinoma and 10 patients were adenocarcinoma;According to the family history,there were 2 patients with family history and 66 patients without family history.There was no significant difference in the general data of the two groups after the statistical test.Compared with the two groups,the bleeding volume(165.71 ± 37.99 ml vs.306.44 ± 75.54 ml P < 0.001),the exhaust time(46.31 ± 4.61 h vs.52.28 ± 6.75 h P < 0.001),the retention time of urinary catheter(12.79 ± 4.94 d vs.14.63 ± 3.01 d P < 0.001).the length of hospitalization(12.56 ± 2.92 d vs.14.43 ± 2.84 d,P < 0.001).The above indexes in laparoscopic surgery group were significantly shorter than those in open surgery group,and were statistically significant.The total operation time(248.56 ± 18.03 mim vs.214.99 ± 27 min,P < 0.001)in the laparoscopic group was longer than that in the open surgery group,the difference was statistically significant.The postoperative complications in the laparoscopic group were 4 cases,significantly lower than that in the open surgery group of 13 cases,(P = 0.032),the difference was statistically significant.The recurrence cases of laparoscopic group was 8 cases,which was significantly higher than that of open surgery group of 1 case,the recurrence rate was higher than open surgery group,the difference was statistically significant.Conclusion:The bleeding volume,the exhaust time,the retention time of urinary catheter,the length of hospitalization and the postoperative complications in laparoscopic surgery group were significantly lower than open surgery group.The total operation time in the laparoscopic group was longer than open surgery group.The average recurrence-free survival in the laparoscopic group was shorter than open surgery group.the recurrence rate was higher than open surgery group.For prognosis,open surgery is better than laparoscopic surgery.
