Surgery has always been the preferred treatment in the comprehensive treatment of gastric cancer,but the traditional open gastric cancer surgery has a relatively slow recovery due to its large damage and long exposure time of abdominal organs.Laparoscopic surgery is more and more widely used in gastric cancer surgery because of its advantages of small trauma,local enlargement and rapid recovery.However,for advanced gastric cancer,whether laparoscopic surgery has advantages and the perioperative effect of laparoscopic gastric cancer surgery are still controversial.I collected the clinical and pathological data of advanced gastric cancer patients treated by general surgery in Sino-Japanese Friendship Hospital of Jilin University from January 2015 to December 2018,and compared the efficacy of laparoscopic and open surgery.Purpose:To compare the clinical effects between the laparoscopic-assisted surgery and open surgery in the treatment of advanced gastric cancer Methods:This retrospective study analyzed data of 80 cases of gastric cancer who undergo open and Laparoscopic-assisted surgery from china-japan union 3rd hospital of Jilin university during January 2015 and December 2018.80 cases of patients were divided into two groups,each group 40 cases,respectively.One group was laparoscopic-assisted surgery,and the next group was open surgery.The pathological stages of all patients were advanced gastric cancer.Those diagnosed with benign lesions or died without surgical related before operation were excluded from the study.Patient's data such as demographic characteristics,the number of total lymph node,operation time,ICU stay time,total hospital stay time,the amount of bleeding,starttime for oral feeding,bowel movement time,the number of complications and pathological results(TNM)were collected.RESULT:The number of total lymph node dissection in laparoscopic-assisted surgery for gastric cancer was larger than that in open surgery for gastric cancer(24.56±11.690 vs.18.22±6.170,P<0.05),but there was no significant difference in the number of positive lymph nodes between the two groups(3.256 ±4.173 vs.3.414±4.097,P>0.05).There was significant difference in operation time,ICU stay time and total hospital stay time between laparoscopic-assisted group and open group(238.15±56.17 min vs.215.45±34.61 min,2.35±0.81 d vs.2.05±0.22 d,9.98±1.52 d vs.12.36±1.16 d,P<0.05).But the amount of bleeding in laparoscopic group was significantly less than that in open group(117.45 ±48.853678)m L vs.(312.33±167.856)m L,P<0.05.There was no significant difference in start time for oral feeding,bowel movement time between laparoscopic-assisted group and open group(6.75±2.10 d vs.6.30±1.42 d,3.40±1.05 d vs.3.95±1.15 d,P>0.05).The number of complications in laparoscopic-assisted surgery group(16 cases)was equal with that in open surgery group(17 cases)(P > 0.05).There was no significant difference in pathological data between the two groups(P>0.05).Conclusion:(1)Laparoscopic-assisted surgery for advanced gastric cancer is helpful to reduce intraoperative bleeding and postoperative complications.(2)Laparoscopic-assisted radical gastrectomy for advanced gastric cancer can achieve the same radical effect as open surgery.
