Effect Of Local Injection Via Bronchoscope On Advanced Lung Squamous Cell Carcinoma

Objective: Exploring the efficacy and safety of local injection of cisplatin combined with Endo under electronic bronchoscope on airway stenosis caused by advanced lung squamous cell carcinoma.Methods: Forty patients with stage III or above lung squamous cell carcinoma admitted to Qingdao Haici Medical Group Pulmonology Department from January 2018 to December 2019 were selected,and they were randomly divided into two groups.On the basis of systemic intravenous chemotherapy combined with radiotherapy,patients in the treatment group were used electronic bronchoscopy to locally inject cisplatin and endol under the microscope.For patients in the control group,they were treated with systemic intravenous chemotherapy combined with radiotherapy.Both groups of patients were treated for two cycles,and each cycle for 21 days.By comparing the bronchoscopy results,CT imaging results,pathological results and adverse reactions of the two groups of patients before and after treatment,a comprehensive evaluation of the short-term treatment effect and safety of local injection treatment with bronchoscope was obtained.Moreover,patients were followed up at 1 and 3 months after treatment to evaluate the short-term efficacy of the two groups.Results: Combined treatment of SqCLC patients with local injection of cisplatin and Endo under the electronic bronchoscope can quickly reduce the size of the tumors in and around the airway and restore the smoothness of the airway in a short time.The effective rate of the treatment group was 85%,and the effective rate of the control group was 50%;three months after the end of treatment,the effective rate of the treatment group was 95%,and the effective rate of the control group was 65%.In terms of scoring,the KPS score and SS score of the two groups of Sq CLC patients were significantly different from that before treatment at 1 and 3 months after treatment(P <0.05);in terms of lung function,the FEV1 results of the two groups of Sq CLC patients were significantly higher than that before the treatment 1 month after treatment,and there was a statistically significant difference(P <0.05).The FEV1 / FVC of the two groups of patients was higher than before treatment 1 month after treatment,and there was a significant difference(P<0.05).Comparing the improvement of lung function between the treatment group and the control group,it was found that the treatment group had better effect on improving lung function than the control group.In terms of safety,there was no significant difference in the incidence of hematological-related adverse reactions between the two groups of patients(P> 0.05).Taking 20 patients in the treatment group,a total of 40 treatment cycles as the observation object,the incidence of adverse reactions of the cardiovascular system before / after treatment: sinus tachycardia 9/18(%),ST-T change12/18(%),Blood pressure fluctuation 5/18(%),clinical symptoms 0/13(%).The new incidence rates were 9%,6%,13% and 13%,respectively.Among them,differences in ST-T changes(P = 0.022),sinus tachycardia(P = 0.039),blood pressure fluctuations(P<0.001)and clinical symptoms(P <0.001)were statistically significant.Conclusion:(1)Local injection of cisplatin and Endo under electronic bronchoscopy can effectively reduce the size of the tumor,restore patients 'airway patency,and improve the quality of life of patients;(2)The short-term effects and prognosis of local injection of cisplatin and Endo under the electronic bronchoscope is good;(3)The treatment of local injection of cisplatin and Endo under the bronchoscope will hardly aggravate the adverse reactions of the patients,and the safety is better.
