Objective:Enterovirus 71(EV71)is a kind of non-enveloped icosahedral,single stranded RNA virus,which belongs to human enterovirus a group.EV71 mainly infects human through fecal oral route,which can lead to typical hand foot mouth disease,encephalomyelitis,brainstem encephalitis,pulmonary edema,poliomyelitis like paralysis and other diseases,and even serious complications such as nervous and mental complications.Since its first discovery,the virus has erupted many times in the Asia Pacific region,causing serious complications and high mortality.EV71 infection mainly depends on multiple factors such as virus,host and environment.The pathogenesis of its serious complications is not clear.It has been shown that multiple cytokine gene polymorphisms are related to EV71 infection,for example,interleukin-17F(IL-17F)is related to EV71 encephalitis,among which IL-17F-1507 gene polymorphisms are related to a variety of immune and infectious diseases,and there is no study on the relationship between EV71 infection severity.In order to further clarify the susceptible population of EV71 and avoid the harm of EV71 severe infection,this paper mainly discusses the correlation between the polymorphism of IL-17F-1507C/ T gene and the severity of EV71 infection.Methods:From June 2014 to June 2018,360 EV71 nucleic acid positive children in the Affiliated Hospital of Qingdao University and Qingdao women’s and children’s hospital were collected as the infection group(189 males,4.28 ± 1.67 years old;171 females,3.84± 1.73 years old).According to the symptoms,signs and relevant auxiliary examinations,the infection group was divided into 233 mild cases and 127 severe cases.350 healthy children(183 males,167 females,5.03 ± 2.12 years old)were selected as the control group.We collected the basic information,clinical characteristics and relevant examination data of all children in the case group and the control group,extracted genomic DNA from EDTA anticoagulant by DNA extraction kit,detected the polymorphism of IL-17F-1507C/T gene locus by multiple high temperature ligase technology,and detected the concentration of serum IL-17 F by ELISA.SPSS 21.0 was used to analyze the experimental data,P < 0.05 showed that the difference was statistically significant.Chi square test was used to compare the qualitative data of each group.Odd ratio and 95% CI(95% confidence interval)were used to reflect the risk of IL-17F-1507.T-test is used to compare the quantitative data of the two groups,and one-way ANOVA is used to compare multiple groups of data.Results:1.The frequency of TT genotype and T allele in EV71 infection group were higher than that in the control group.The frequency distribution of TT genotype in EV71 infection group was statistically significant(P = 0.047,OR = 1.640,95% CI =1.026-2.621),and the frequency of T allele in EV71 infection group was statistically significant(P = 0.01,OR = 1.329,95% CI = 1.072-1.648).2.The frequency of TT genotype in EV71 severe infection group was about 2 times higher than that in mild infection group,and there was statistical difference between the two groups(P = 0.007,OR = 2.257,CI = 1.246-4.088).The distribution of allele frequency in severe group was higher than that in light group(P = 0.003,OR = 1.615,CI = 1.185-2.202).3.Among the three genotypes of CC,CT and TT,there was no significant difference in gender(P = 0.069),age(P = 0.088),creatine kinase isoenzyme(P = 0.165),alanine aminotransferase(P = 0.453),aspartate aminotranferase(P = 0.186).There were significant differences in fever duration(P = 0.008),leukocyte count(P = 0.001),C-reactive protein(P = 0.006),blood glucose(P = 0.023)and other laboratory tests.4.The concentrations of IL-17 F were 50.21±15.89pg/ml,70.54±24.33pg/ml,102.57±29.53pg/ml in the control group,mild infection group and severe infection group,respectively.The concentration of IL-17 F in severe infection group was higher than that in mild infection group and control group(P < 0.001),while that in mild infection group was higher than that in control group(P < 0.05).The concentrations of IL-17 F in three genotypes of severe EV71 infection group were 72.79±13pg/ml in CC type,103.6±21.65pg/ml in CT type and 135.86±25.01pg/ml in TT type.In severe infection group,the serum IL-17 F concentration of TT type was higher than that of CT type(P < 0.05),TT type was higher than CC type(P < 0.001),and CT type was also higher than CC type(P < 0.05).There was no significant difference in the serum IL-17 F concentration among the three genotypes in mild infection group.Conclusion:1.IL-17F-1507 C / T gene polymorphism is related to EV71 severe infection.2.IL-17F-1507C/T genotype TT and allele T may be the risk factors of EV71 serious infection.
