Objective This study was designed to investigate the possible association of IL-4-589C/T gene polymorphism and severity of EV71 infection. Methods The IL-4-589C/T gene polymorphism among EV71-infected cases(n=185), including mild cases(n=102) and severe cases(n=83) as well as healthy controls(n=234) was done by the improved multiplex ligation detection reaction(iMLDR) technique after the extraction of genomic DNA with peripheral blood leukocytes. The plasma levels of IL-4 and IFN-γ were determined by enzyme-linked immunosorbent assays. Record the patient’s general condition, clinical presentation, and the necessary laboratory testing index. Results 1. There are no statistically significant differences in genotypes and allele frequencies of the IL-4-589C/T gene cite(P>0.05). 2.The presence of CC genotype was significantly higher in severe cases(10.8%)than that in mild cases(4.9%,P<0.05). Furthermore, in EV71 encephalitis, CC genotype(15.8%) were also more frequent(P<0.05). 3. The presence of C allele was significantly higher in severe cases(27.7%)than that in mild cases4.9%(15.2%,P<0.05,OR=2.1, 95%CI 1.3-3.6). Furthermore, in EV71 encephalitis, C allele(31.6%) were also more frequent(p<0.05,OR=2.6, 95%CI 1.4-4.8). 4. Significant differences were observed in EV71 infected patients in different genotypes with respect to duration of fever, white blood cell counts, C-reactive protein levels and blood glucose concentrations(P<0.05). 5. The plasma levels of IL-4 of CC( 8.0±1.3 pg/m L, p<0.05)and CT genotype(6.8 ±2.1 pg/mL, P<0.05) were significantly elevated compared to TT genotype(4.6 ±1.2 pg/mL). But the plasma levels of IFN-γ and the IFN-γ/IL-4 ratio were significantly decreased in CC and CT genotype in comparison with TT genotype( P<0.05). Conclusion 1. IL-4-589C/T gene polymorphism may has no relation with the susceptibility of EV71 infection. 2. The elevation of IL-4 production may lead to the development of severe EV71 disease with allele C.
