ObjectiveTo develop a Self-management Assessment Scale for gout patients in China,and test the reliability and validity of the scale.In order to evaluate the level of gout patients' self-management,to understand their self-management needs,and to verify the effectiveness of related interventions.Methods1.Retrieval strategy was used to search databases at home and abroad,review the domestic and foreign literature related to gout self-management,self-management scale and self-management theory,and develop a hierarchical measurement model of gout patients' self-management.2.On the basis of literature research,we interviewed gout patients and medical staff,collected their suggestions on gout self-management as an important basis for the development of the scale,and initially constructed the item pool of the scale.3.Delphi method was used to conduct expert inquiry on the item pool of the preliminary scale,and the items of the scale were screened for the first time.After pre investigation,the pre-test scale of gout patients' self-management evaluation was formed.4.Using self-management pre-test scale to design the questionnaire,the first on-site questionnaire survey was carried out for gout patients,and the formal scale was formed after the items of the scale were screened again according to the survey results.The second field survey was carried out using the questionnaire designed by the formal scale to test the reliability and validity of the scale.Results1.53 effective literatures related to gout self-management,self-management scale and self-management theory were obtained.Based on the theory of three tasks and five skills of self-management,the hierarchical measurement model of the scale constructedby combining the characteristics of gout disease includes three first level indicators and11 second level indicators.2.A total of 10 gout patients and 5 gout related medical staff were interviewed.Based on the results of the interview,5 topics related to gout patients' self-management were extracted,and 59 items of item pools were initially established.3.32 and 30 questionnaires were sent out for the two rounds of expert correspondence,and 30 questionnaires were recovered,with the effective recovery rate of93.75% and 100% respectively.In the first round,17 experts proposed changes,and in the second round,6 experts proposed changes.The two rounds of expert authority coefficients are 0.850 and 0.865 respectively.Kendall's W was 0.350 and 0.438 respectively.After two rounds of expert correspondence,the second dimension was selected from "lifestyle management".16 items were deleted,11 items were merged,and12 items were added.The pre-test scale consisted of 4 dimensions and 49 items.In the pre survey,it was determined that Likert grade 5 scoring method was used to answer the scale.4.After the first field survey,8 items were deleted and merged after project analysis.The formal scale consists of 4 dimensions and 41 items.The reliability and validity test results of the formal scale of the second field survey show that Cronbach's ? coefficient of the total scale is 0.962,half reliability is 0.842,and retest reliability is 0.904 in two weeks.The content validity index(I-CVI)of item level was 0.875-1,and the content validity index(S-CVI)of scale level was 0.951.Four common factors were extracted by exploratory factor analysis,the cumulative contribution rate of variance was 60.779%,and the common variance of items was greater than 0.4,between 0.617-0.821.ConclusionThe Self-management Assessment Scale for gout patients has been developed in this study.The scale has good reliability and validity,which can be used as a measurement tool to assess the self-management ability of gout patients.
