Objective1.To translate and revise the Prognosis and Treatment Perception Questionnaire(PTPQ),and test its reliability and validity.2.To explore the prognosis and treatment perception status of advanced cancer patients,and analyze the influencing factors to provide a reference basis for later clinical intervention.Methods1.Formation and reliability and validity test of Chinese version of PTPQ: The English version of PTPQ was translated,back-translated,cultural modified and revised based on Brislin translation model,so as to form the Chinese version of PTPQ.The Chinese version of PTPQ was applied to 198 patients with advanced cancer.Item homogeneity test and item discrimination test were used to analyze the items of the questionnaire;The validity of the questionnaire was evaluated by content validity and structure validity;The reliability of the questionnaire was evaluated by calculating the internal consistency reliability(Cronbach's ? coefficient)and Guttman's half-reliability.2.Analysis of current status and influencing factors of prognosis and treatment perception of patients with advanced cancer: General information questionnaire,Chinese version of PTPQ,Brife illness perception questionnaire(BIPQ),Chinese version of Herth Hope scale and Hospital anxiety and Depression scale(HADS)were used to investigate the 287 advanced cancer patients who was conducted from five hospitals in two tertiary hospitals in Qingdao by convenient sampling.Descriptive statistical analysis,chi-square test,t-test,one-way ANOVA,correlation analysis,binary Logistic regression analysis and multiple linear regression analysis were used to analyze the statistical data.Results1.Formation and reliability and validity test of Chinese version of PTPQThe Chinese version of PTPQ includes two sub-questionnaires,a total of 17 items,of which the first sub-questionnaire includes five items,which are in the form of single selection,and the second sub-questionnaire includes 12 items,which are scored in the form of 11 level semantic difference(0?10).Item analysis showed that the correlation coefficient between the items in the second sub-questionnaire and the total score of the questionnaire was 0.419 ? 0.745,all of which were > 0.4;There were significant differences in the scores of each item between the high score and the low score(P<0.001).Validity analysis showed that the content validity index(I-CVI)of each item in the Chinese version of PTPQ was 0.80?1.00,and the content validity index(S-CVI)of the questionnaire level was 0.91.The second sub-questionnaire extracted four common factors through exploratory factor analysis,and the contribution rate of cumulative variance was 82.513%.The reliability analysis showed that the Cronbach's ? coefficient of the total questionnaire was 0.845,the Cronbach's ? coefficient of the first sub-questionnaire was 0.904,the Cronbach's ? coefficient of the second sub-questionnaire was 0.871,the Cronbach's ? coefficient of the four factors extracted was 0.688?0.956,and the Guttman's half-reliability coefficient of the total questionnaire was 0.867.1.Analysis of current status and influencing factors of prognosis and treatment perception of patients with advanced cancer(1)Analysis of treatment perception and influencing factors of advanced cancer patients: In this study,61.0% of the patients with advanced cancer had too ideal and optimistic attitude towards the outcome of their treatment,and still had high expectations for the cure of cancer,and the perception of treatment was not accurate.The results of binary Logistic regression showed that marital status,whether the patient had received radiotherapy or chemotherapy,the time since the initial diagnosis,the illness perception and the level of hope were the influencing factors of the treatment perception of patients with advanced cancer.Among them,no spouse(OR=10.240,95%CI=1.155 ?90.755),radiotherapy and chemotherapy(OR=6.004,95%CI=3.115?11.570),and high hope level(OR=1.095,95%CI=1.019 ? 1.177),after the initial diagnosis ?1 year(OR=3.583,95%CI=1.983 ? 6.475)were risk factors for patients with advanced cancer to report inaccurate treatment perception.High perceived threat of illness(OR=0.941,95%CI=0.905 ? 0.979)was a protective factor for patients with advanced cancer who reported inaccurate treatment perception.(2)Analysis of the prognosis perception(degree of attention to prognosticinformation)and its influencing factors in patients with advanced cancer: The score of prognosis perception(degree of attention to prognostic information)of advanced cancer patients was(88.00±14.05),and the score of the item on the hospice discussion was the lowest,which was(4.91±2.58).The results of multiple linear regression showed that age,education level,illness perception,anxiety,depression and hope level were the influencing factors of prognosis perception(degree of attention to prognosis information)of advanced cancer patients,accounting for 75.1% of the variation.Conclusion1.The Chinese version of PTPQ has good reliability and validity and meets the requirements of metrology.It is suitable for the evaluation of prognosis and treatment perception of advanced cancer patients in China.Chinese medical staff can use it to evaluate the prognosis and treatment perception of advanced cancer patients.2.The prognosis and treatment perception of advanced cancer patients are affected by many factors and have individual differences,suggesting that clinical medical staff should pay attention to the prognosis and treatment perception of advanced cancer patients.And according to the characteristics of patients to formulate corresponding intervention measures to improve the prognosis and treatment perception of patients,in order to promote their end-of-life decision-making.
