Objectives1.To develop a work pressure scale for the postpartum nurses who returned work and to test its reliability and validity;2.To investigate the current status of work pressure in the postpartum returning nurses in tertiary general hospitals by the scale and analyze the influencing factors.Methods1.The original items and a first draft of the scale were based on the theory of"Cognitive-Phenomenological-Transactional"and collected through literature review,semi-structured interviews and group discussions.The Delphi technique was used to further adjust and screen the scale entries to form a beta version of the scale.2.The scale was selected by the item analysis and exploratory factor.A total of 635postpartum nurses returning from 6 tertiary general hospitals were conveniently selected,and the reliability and validity of the scale were tested using content validity index,standard validity,structural validity,internal consistency,and split-half reliability.3.A total of 1,461 postpartum nurses in 38 tertiary general hospitals nationwide were selected by the convenience sampling method.The general information questionnaire,the work pressure scale for the postpartum nurses who returned work and the self-rating depression scale were used as research tools.The statistical methods including descriptive statistics,t test,single factor analysis,and multivariate analysis were used to analyzed the current status of work pressure among the postpartum returning nurses and its influencing factors.Result1.Combining the results of literature review,semi-structured interviews and group discussions,the draft scale contains 41 items.24 experts were selected to conduct two rounds of Delphi expert consultations to screen and adjust the scale items.The effective recovery rates of the two rounds of consultations were 100%and 87.50%respectively,the opinion submission rates were 62.50%and 19.00%respectively;the authoritative coefficients are 0.847 and 0.888;the expert coordination coefficients are 0.138 and 0.238(P<0.01).After the consultation,a total of 10 items were deleted,and 6 items were modified.The resulting scale test version contains 30 items belonging to 5 dimensions.2.After items analysis and exploratory factor for item screening,a total of 3 items were deleted,and a total of 5 common factors were extracted by exploratory factor analysis.The cumulative contribution rate was 59.379%.The resulting scale contains 5dimensions and 30 items.After the reliability and validity test,the content validity of the scale was 0.943;the correlation coefficients with the self-rating depression scale and the Mc Closkey/Mueller satisfaction scale were 0.572 and-0.482(P<0.01);the confirmatory factor analysis confirmed that the 5-factor model fitted well(c~2/df=2.924,RMSEA=0.055,GFI=0.889,NFI=0.891,IFI=0.925,TLI=0.916,CFI=0.925);The Cronbach's?coefficient was 0.940,the split-half reliability was 0.882 for the total scale.3.The total work stress of postpartum return nurses was(87.07±19.59),the highest score was“mother role commitment”,and the lowest score was“personal relationship”;multiple linear regression analysis showed the title,department,depression status,returning time,maternity leave time and the parity of the nurses were the influencing factor of the post-partum return nurses'work stress(P<0.05).Conclusion1.The work pressure scale for the postpartum nurses who returned work has good validity and reliability,which can be applied to measure postpartum nurses'work pressure.2.The work pressure on postpartum return nurses in tertiary hospitals is at an above medium level.Nursing managers should pay attention to the postnatal nurse group and develop targeted measures to reduce postpartum return nurse's work pressure.
