Objective:To investigate the antibiotic resistance and the risk factors of infection of(ESBLs)infection in elderly patients and provide the basis for the treatment,prevention and rational use of Escherichia coli infection in elderly patients.Methods:The clinical data and in vitro results of ESBLs and non-ESBLs producing Escherichia coli were collected from January 2019 to December 2019 in our hospital,and the distribution characteristics of the two groups of patients and their resistance to various antibiotics were analyzed.The risk factors of ESBLs producing Escherichia coli infection were analyzed by?~2 test and multi-factor logistic regression analysis.Results:A total of 615 strains of Escherichia coli were isolated from January 2019 to December 2019 among various specimens of elderly patients in our hospital,of which305 were ESBLs producing Escherichia coli with a detection rate of 49.59%and 310were non-ESBLs producing Escherichia coli with a detection rate of 50.41%.The highest separation rate of ESBLs producing Escherichia coli and non-ESBLs producing Escherichia coli were urine,followed by blood and drainage fluid,which were more common in general surgery ward and urology ward.The drug resistance of ESBLs producing Escherichia coli isolated from older patients to most of the cephalosporins,fluoroquinolones,aminoglycosides and so on were higher than that of non-ESBLs producing Escherichia coli(P<0.05).Hospital staying over 14 days,antibiotic using more than 7 days,indwelling catheterization,indwelling drainage tube,the surgery and trauma were all independent risk factors for infection of ESBLs producing Escherichia coli in elderly patients.Conclusions:ESBLs producing Escherichia coli in older patients had a higher rate of drug resistance.Avoiding exposure to risk factors such as long hospitalization time,long antibiotic using time,indwelling catheterization,indwelling drainage tube,surgery and trauma,and rational use of antimicrobial agents can effectively prevent the occurrence ofESBLs producing Escherichia coli infection in elderly patients.
