Prognosis Of Stroke Patients In Rural Liaoning

Objective: Although China has made important progress in stroke prevention and treatment strategies,it is still the leading cause of death in China.In rural China,both morbidity and mortality are higher than those in cities.Therefore,we aim to understand the prognosis of stroke patients in rural Liaoning.Method: This study is a prospective cohort study.The baseline survey was conducted from 2004 to 2006.Three follow-up surveys were completed in December 2008,December 2010,and December 2017.A total of 1,418 participants were included in the study.Adverse events included relapse,all-cause mortality,and CVD mortality.Differences between groups were compared using analysis of variance or Kruskal-Wallis H test.Use the number of events per 1,000 person-years to represent the event rate and understand the event.The life-table method was used to estimate the cumulative survival rate of different endpoint events.Result: In this study,the incidence of adverse events was 43.46/1000 person-years,the recurrence rate was 32.27/1000 person-years,all-cause mortality was 30.77 / 1000 person-years,and CVD mortality was 23.70/1000 person-years.The incidence of adverse events was 49.53/1000 person-years for males and 35.66/1000 person-years for females.The incidences of adverse events of hemorrhagic,ischemic,and unclassified stroke were 57.04/1000 person-years,43.56/1000 person-years,and 22.05/1000 person-years,respectively.The 10-year cumulative survival rate for male patients was 62.8%,and the cumulative survival rate for female patients was 72.5%.The cumulative survival rates of hemorrhagic,ischemic,and unclassified stroke were 59.4%,67.0%,and 81.4%,respectively.Multivariate analysis results show that age,males,SBP,and diabetes can increase the risk of adverse events,and the course of disease,high physical activity,and high blood lipids reduce the risk of adverse events.Conclusion: Ischemic stroke accounts for a higher proportion of stroke patients in rural Liaoning,but patients with hemorrhagic stroke have a worse prognosis.Males and patients with hemorrhagic stroke have a higher risk of recurrence and death,which prompts us to pay more attention to males and patients with hemorrhagic stroke.For stroke patients,we should invest more human,material and financial resources to improve their prognosis.
