Objective:To obtain the epidemiological status of overweight and obesity of infants in Shenyang,and explore the relationship between behavioral factors such as diet and physical activity during pregnancy and overweight and obesity among infants,so as to provide clues to disease for the early prevention of overweight and obesity of children.Methods:By setting up the prospective birth cohort study,the pregnant women who met the inclusion criteria were included in the baseline by stratified sampling in five administrative regions of Shenyang City,and were followed up at the birth of children,1-month-old,3-month-old,6-month-old,8-month-old and 12-month-old respectively.The general demographic information and obstetric examination information of pregnant women and their families were collected by questionnaire.The Food Frequency Questionnaire and 24 Hours Meal Review were used to investigate the pregnant women's dietary status,and Pregnancy Physical Activity Questionnaire was used to investigate the pregnant women's physical activity.We also standardized measured infant's body length,weight and other indicators.EpiData v3.1 was used to input and establish the database,StataSE v15.1 was used for statistical analysis,factor analysis was used to calculate the pregnant women's dietary pattern,multiple linear regression,logistic regression and mixed effect model were used to analyze the correlation between the pregnant women's dietary behavior,physical activity and their offspring's overweight and obesity.Results:1.In 1070 infants,the incidence of macrosomia and large for gestational age was 10.50% and 36.76%,respectively.The incidence of overweight at 6 months and 12 months was 7.09% and 7.03%,respectively.And the detection rate of overgrowth at 0-6 months and 0-12 months was 47.72% and 45.34%,respectively.2.The dietary pattern of mothers during pregnancy included fish and seafood diet pattern,high protein diet pattern,traditional diet pattern,high sugar diet pattern,cereal-seafood diet pattern.There was a significant negative correlation between maternal high protein diet pattern score and BMI z–score of offspring aged 0-12 months(?:-0.039,95%CI:-0.078?-0.002).The risk of large for gestational age with higher protein diet pattern score(Q2)was 60%(OR: 0.60,95% CI: 0.41?0.87)of that with lower high protein diet pattern score(Q1).The risk of overweight in 6-month-old offspring with higher protein diet pattern score(Q3)was 38%(OR:0.38,95%CI:0.15?0.94)of that with lower protein diet pattern score(Q1).There was a significant positive correlation between traditional diet pattern score and BMI z-score(?: 0.042,95%CI: 0.001?0.082).The risk of overweight in 6-month-old offspring with higher traditional diet score(Q3)was 2.76(OR: 2.76,95%CI: 1.15?6.58)times higher than that with lower traditional diet score(Q1).There was a significant positive correlation between traditional diet pattern score and the growth rate of 0-6 months old of offspring(?: 0.061,95%CI: 0.001?0.121).The risk of overgrowth with higher traditional diet score(Q4)was 1.89(OR:1.89,95%CI: 1.07?3.33)times higher than that with lower traditional diet score(Q1).3.There was no significant correlation between the total physical activity level during pregnancy and the related indicators of overweight and obesity in offspring.The risk of overweight in 6-month-old offspring with higher resting physical activity level(Q3)was 2.56 times higher than that with lower resting physical activity level(Q1),but it was not related to overweight at 12 months.There was no significant correlation among other physical activity types.Conclusion:The diet behavior of mothers during pregnancy is related to overweight and obesity of infants,while physical activity is not significantly related to overweight and obesity of infants,which needs further exploration and verification.The behavioral factors in pregnancy are very important for the prevention of overweight and obesity in children.We should further strengthen the early life prevention and control measures for childhood obesity.
