Objective: We are going to investigate the relationship between preterm premature rupture of membranes(p PROM)and periodontal diseases by comparing the periodontal conditions and serum levels of IL-6,CRP and PCT in pregnant women with p PROM and those are none preterm rupture of membranes(n PROM).Methods: In this study,40 cases of p PROM and 40 cases of n PROM in the maternity ward of Shengjing hospital affiliated to China medical university were selected.Clinical periodontal examination was performed to determine whether the pregnant women had periodontitis,which is one of the main types of PD.Among them,22 cases of p PROM and periodontitis(group A);18 cases of p PROM without periodontitis(group B);12 cases of n PROM with periodontitis(group C);28 cases of n PROM without periodontitis(group D).Pregnant women with p PROM were selected from 30 to 37 weeks of gestational age.The diagnosis of p PROM was based on the American college of obstetricians and gynecologists' guidelines for premature rupture of membranes(2018).In other words,the pre-labor amniotic fluid was discharged through the vagina before 37 weeks of gestation,and the PH value of the vaginal fluid was alkaline.Or the vaginal fluid smear was examined microscopically after drying,and there were ferns crystals under the microscope.The n PROM group selected healthy pregnant women at 37-40 weeks gestation without premature rupture of membranes and other systemic diseases.Periodontitis is diagnosed according to the new international classification of periodontitis and periphytic disease in 2018.The subjects should meet the following characteristics at the same time 1.Periodontal pocket>3mm with inflammation;2.Adjacent clinical attachment loss>1mm;3.Bleed after probing.Patients who met all three criteria were included in periodontitis groups.Results: The prevalence of periodontitis in p PROM pregnant women was higher than that in n PROM pregnant women(c2=5.115,P=0.024).PD(P=0.001)and SBI(P=0.037)in group A were significantly higher than those in group C.PD of group B was significantly higher than that of group D(P=0.011).There was no statistically significant difference in PLI between groups A and C,B and D,C and D,with P values of 0.292,0.532 and 0.060,respectively.The levels of IL-6(P=0.002),CRP(P=0.016)and PCT(P=0.009)in group A were significantly higher than that in group B,and the levels of IL-6(P=0.009)and CRP(P=0.002)in group C were significantly higher than that in group D.There was no statistically significant difference in PCT levels among other groups except for group A and B(P >0.05).Conclusion: p PROM may indicate a higher prevalence of periodontitis.The host's immune response may be one of the key pathways through which Periodontitis and p PROM interact,and IL-6 and CRP may be crucial in this process.
