Objective: To evaluate the efficacy of catheter ablation in patients with refractory atrial fibrillation(AF)and to explore the risk factors for recurrence within 1 year.Methods: 74 patients who received radiofrequency ablation in the first Department of Cardiology,Shengjing Hospital affiliated to China Medical University from January 2013 to December 2017 were retrospectively selected to discuss the factors related to the recurrence of ATRIAL fibrillation within 1 year after surgery.All patients must be confirmed as atrial fibrillation by at least one electrocardiogram or 24-hour dynamic electrocardiogram before surgery,relevant preoperative examination should be completed,and thrombosis of the left atrial and auricle was excluded by transesophageal ultrasonography.If ultrasonography confirms the presence of thrombus,oral anticoagulants will be administered until the thrombus disappears and surgery is selected.All the 74 patients with ATRIAL fibrillation completed circum-pulmonary vein electrical isolation,and received bilateral and bilateral anterior vestibular great ring linear ablation.The end point of the ablation was two-way electroblock of pulmonary vein and left atrium,and the immediate ablation success rate was 100%.Based on the different onset after 1 year,51 patients(78.48%)maintained sinus rhythm(no recurrence)and 14 patients(21.52%)relapsed(recurrence).Common clinical indicators were included,such as gender,age,atrial fibrillation type,left atrial diameter,ejection fraction,cardiac function grade,duration of disease,coronary heart disease,other arrhythmias,diabetes,hypertension,and ACEI/ARB drug use.The follow-up was completed in January 2019,and the postoperative observation time of all included patients was more than 1 year.According to the follow-up time,the patients were divided into two groups according to the occurrence of rapid atrial arrhythmia(atrial tachycardia,atrial flutter and atrial fibrillation)1 year after the operation.The clinical data of the two groups were statistically analyzed by single factor,and then the independent risk factors for the late recurrence of ATRIAL fibrillation were obtained by using Logistics multi-factor analysis.Results: 1.According to statistics,14 patients(21.52%)had recurrent ATRIAL fibrillation.2.Basic information of the two groups of patients Univariate risk factor analysis of clinical data showed that the duration of disease and the use of ACEI/ARB drugs were statistically significant compared with the recurrence of ATRIAL fibrillation(P<0.05).3.According to Logistics multi-factor analysis,the risk factors were duration of disease[OR=2.502,95%CI: 1.228-5.098,P<0.05].Conclusions: 1.Compared with the non-recurrent group,recurrent atrial fibrillation within 1 year after transcatheter ablation was associated with the duration of disease and the use of ACEI/ARB drugs.2.Duration of disease is a risk factor for recurrence 1 year after catheter ablation.
