Objective: To investigate the efficacy and safety of 125I radioactive seed implantation combined with vertebroplasty?PVP?in the treatment of vertebral metastases.Methods: A retrospective analysis of 18 patients with thoracolumbar metastases,each patient had at least 1 vertebral body compression changes with varying degrees of pedicle or vertebral posterior margin or paravertebral involvement,under DSA guidance Percutaneous pedicle puncture vertebroplasty combined with 125I seed implantation;follow-up 2 months after surgery,by comparing patients with preoperative and postoperative pain relief?VAS score?and daily behavioral improvement?KPS score?To evaluate the efficacy of surgery,to evaluate the safety of surgery by observing the presence of serious complications during and after surgery.The follow-up interval was 3days after surgery,1 month after surgery,2 months after surgery,preoperative and surgery.Differences in follow-up results at different post-stages were assessed by one-way analysis of variance.Results : A total of 18 patients in this study group received a total of 18 vertebral bodies treated with 125I radioactive particles combined with vertebroplasty.The preoperative average VAS score was 6.89±0.87 points?1-8 points?and 3.67±0.94 points after three days.?1-8 points?,2±0.75 points?1-8 points?in the first month after surgery,and1.78±0.63 points?1-8 points?in the two months after operation.The statistical analysis showed that the VAS was followed up in each period after operation.There was a statistically significant difference between the scores and preoperative scores?P < 0.001?.The preoperative average KPS score was 61±13 points?30-90 points?,66±13 points?30-90 points?for three days after surgery,and 72±13 points?30-90 points?for one month after surgery.The last 2 months were 69±14 points?30-90 points?.The statistical analysis showed that the difference between the preoperative and postoperative 1 months was statistically significant?P<0.005?,but before and 3 days after surgery.The 2-month follow-up score was not statistically significant,but none of the 18 patients had a KPS score that was lower than that before surgery.All patients had no adverse events related to puncture during and after operation,and no complications such as osteomyelitis,pulmonary embolism,and cardiopulmonary failure.CT scans at 2 months postoperatively showed no significant progression in the tumor area of the surgical vertebral body.Conclusion: Percutaneous kyphoplasty combined with 125I radioactive particles is a safe and effective method for the treatment of vertebral metastases?especially vertebral bodies with posterior margin and vertebral attachment destruction?.
