| Objective:To summarize the clinical characteristics of heart failure,the syndrome differentiation,and the relevant laws of prescriptions for the treatment of heart failure by summarizing the clinical data of patients with heart failure in the third department of cardiovascular disease in the First Affiliated Hospital of Heilongjiang University of Traditional Chinese Medicine.More ideas and directions.Methods:The data included in this study were derived from outpatients and inpatients in the Third Department of Cardiovascular Diseases of the First Affiliated Hospital of Heilongjiang University of Chinese Medicine from September 2017 to September 2 019.The age,gender,basic diseases,western medicine,and traditional Chinese medicine of these patients were counted.Relevant information such as syndrome type,tongue and vein distribution,season of onset,and medication selection should be analyzed retrospectively.Result:1.Of the 166 patients,81 were male(48.80%)and 85 were female(51.20%).The number and percentage of men and women in each age group are as follows:20 cases(12.05%)of patients 45-54 years old,15 cases(75.00%)of males,5 cases(25.00%)of females;4 7 cases(28.31%)of 55-64 years old patients,20 males(42.55%),27 females(57.45%);60 patients(36.14%)65-74 years old,28 males(46.67%),32 females(53.33%);39 patients over 75 years old Cases(23.49%),18 males(46.15%)and 21 females(53.85%).The proportion of male patients in the 45-54 age group is significantly higher than that of females,while in the other three ages,there is no significant difference in the prevalence of males and females.2.Statistics of relevant basic diseases used in the selected patients.The top three basic diseases are 147 cases of coronary heart disease(88.55%),105 cases of hypertension(63.25%),and 91 cases of atrial fibrillation(54.82%)3.The top three frequencies of western medicine use were diuretics in 141 cases(84.94%),ACEI/ARB in 137 cases(82.53%),and nitrate in 124 cases(74.70%)4.The statistics of TCM syndrome types of the selected patients are as follows:35 cases of cardiopulmonary qi deficiency syndrome(2 1.08%),12 cases of heart-kidney yang deficiency syndrome(7.23%),23 cases of yang deficiency and water flooding syndrome(13.86%),and 37 qi-yin deficiency syndrome 37 Cases(22.29%),2 9 cases of phlegm drinking and obstructing lung syndrome(17.47%),30 cases of Qi deficiency and blood stasis syndrome(18.07%);TCM syndrome types with the highest proportion of heart function in the Ⅱ,Ⅲ,and Ⅳ groups were cardiorespiratory qi deficiency syndrome There were 23 cases(29.49%),14 cases of deficiency of qi and yin(23.73%),and 14 cases of deficiency of yang and water(48.28%).5.The statistics of tongue veins of the selected patients are as follows:48 cases with pale tongue,37 cases with red tongue,32 cases with purple tongue,37 cases with petechiae and petechiae;22 cases with yellow tongue coating,67 with white tongue coating,and 44 with greasy tongue coating.15 cases with less tongue fur;31 cases with thin pulses,26 cases with pulse count,31 cases with pulse deficiency,28 cases with slippery pulses,18 cases with pulse pulsations,25 cases with astringent pulses,20 cases with delayed pulses,and 21 cases with pulse nodes6.The incidence season of selected patients is as follows:46 cases(27.71%)in spring,20 cases(12.05%)in summer,39 cases(23.49%)in autumn,and 61 cases(36.75%)in winter;the incidence is classified according to the 2 4 solar terms The first six are Lichun,Osamu,rain,winter solstice,light snow and frost 7.The top 15 Chinese medicine use frequencies of the selected patients are:Poria,Radix Glycyrrhizae,Codonopsis,Schisandra,Ophiopogon,B aiji,Atractylodes,Angelica,Astragalus,Coix Seed,Fritillaria,Chuanxiong,Huangdi,Tangerine,Pinellia8.The top ten frequency of use of the basic prescription of selected patients are as follows:Shengmai San 82 times(12.20%),Erchen Decoction 76 times(11.31%),Yiqi Bufei Decoction 62 times(9.23%),Siwu Decoction 57 times(8.48%),Zhenwu decoction 52 times(7.74%),Baoyuan decoction 43 times(6.40%),Lingguizhugan decoction 38 times(5.65%),Jujube Xiefei Decoction 2 9 times(4.32%),blood Fu Zhuyu Decoction 19 times(2.83%),Yupingfeng scattered 19 times(2.8 3%).Conclusion:1.The prevalence of men in the 45-54 age group is significantly higher than that of women,and there is no significant difference in the prevalence of men and women over the age of 54;the most common underlying diseases of heart failure are coronary heart disease,hypertension and atrial fibrillation;diuretics,ACEI/ARB,Aldosterone receptor antagonists,β-receptor blockers and nitrate drugs are commonly used drugs for the treatment of heart failure,and the use rate of digitalis drugs is low2.The classification of heart function in patients with heart failure is correlated with TCM syndromes.In heart function levelⅡ,cardiopulmonary qi deficiency is the most common type,levelⅢ is mostly due to deficiency of both qi and yin,and level Ⅳ is mostly due to deficiency of yang and water.The number of grades gradually increased,and the distribution ratio of each TCM syndrome type of patients gradually changed,which was in line with the pathogenesis of the disease3.In patients with heart failure,pale tongue and white fur are the most common,and the most common pulse is weak and thin4.The incidence of heart failure is related to the season,and the incidence is higher in winter and spring5.Heart failure medications are related to seasons.Angelica,Ligusticum chuanxiong,Fangfeng,Peppermint and B upleurum are used more in spring,B aiziren,Danshen,Coptis and Gardenia are used more in summer,American ginseng,Fritillaria,Ophiopogon and Lily It is used more in autumn,and turtle shell,Morinda officinalis,cinnamon and aconite are more used in winter6.Prescriptions for the deficiency of heart failure mainly use tonics such as S hengmai San,Yiqi Bufei Decoction,B aoyuan Decoction,Yupingfeng Powder,Siwu Decoction,etc.The standard is to remove blood stasis,resolve phlegm,and to drink Mainly,such as Xuefu Zhuyu Decoction,Zhenwu Decoction,Tingli Dazao Xiefei Decoction,Linggui Shugan Decoction,Erchen Decoction,etc. |
