Objective: To study the changes of serum TNF-? and leptin levels in patients with liver cirrhosis and to explore their significance in the pathogenesis of liver cirrhosis.Methods: From December 2018 to December 2019,50 inpatients with cirrhosis were collected from the First Affiliated Hospital of Jiamusi University,including 28 males and 22 females,aged 35-68 years old,with body mass index(BMI)(22.51 ± 3.77)kg / m2,including 35 patients with hepatitis B cirrhosis and 15 patients with alcoholic cirrhosis.According to the Child-Pugh score,the cirrhosis group was divided into group A,B and C.A total of 30 healthy people were selected as the control group,including 16 males and 14 females,42-56 years old,with body mass index(BMI)(22.66 ± 4.08)kg / m2.The indexes of biochemical and coagulation function were collected from patients with liver cirrhosis and healthy people under fasting condition.The severity of liver cirrhosis and the presence of esophageal varices were assessed by abdominal color Doppler ultrasound,spiral CT and electronic gastroscope.The changes of TNF-? and leptin were measured by ELISA.According to the statistical software spss23.0,the results of index comparison among each group,TNF-?,serum leptin and their correlation analysis.Result:(1)The levels of TNF-? and leptin in the cirrhosis group were significantly higher than those in the control group(P < 0.05);(2)The level of TNF-? in the blood of patients with child Pugh grade C cirrhosis was higher than that of children Pugh grade B(P < 0.05);the level of leptin in the blood of patients with child Pugh grade C cirrhosis was higher than that of children Pugh grade A(P < 0.05);the level of child Pugh grade C cirrhosis was higher than that of children Pugh grade B(P < 0.05).(3)In the cirrhosis group,there was no significant difference in TNF-? level between alcoholic cirrhosis and hepatitis B cirrhosis(P > 0.05);the serum leptin level of alcoholic cirrhosis was higher than that of hepatitis B cirrhosis(P < 0.05).(4)The levels of TNF-? and serum leptin in cirrhotic patients with esophageal varices were significantly higher than those without esophageal varices(P < 0.05).(5)The levels of TNF-? and serum leptin were positively correlated with child Pugh score(P < 0.05),and TNF-? was positively correlated with serum leptin level(P < 0.05).Conclusion:1.The levels of TNF-? and leptin in the blood of patients with cirrhosis were higher than those of patients with alcoholic cirrhosis.2.The level of TNF-? and serum leptin is related to the degree of liver damage.The more serious the degree of liver damage is,the higher the level of TNF-? and serum leptin is.3.TNF-? and serum leptin were positively correlated.
