ObjectiveTo evaluate the diagnostic value of the modified ABCD2 score scale for patients with isolated posterior circulation ischemia vertigo,and analyze the related risk factors and influencing factors of prognosis,so as to provide reference data for early clinical identification and prevention of isolated posterior circulation ischemia vertigo in the future,and lay a theoretical foundation for predicting the possible outcome and formulating treatment strategies.MethodThis paper belongs to a retrospective study.By setting the inclusion and exclusion criteria,the clinical data of 235 patients with acute vertigo /dizziness who were admitted into the Department of Neurology of Xiangdong Hospital Hunan Normal University from January 1,2018 to August 31,2019 were finally screened out and collected.The age,gender,headache,stroke history,hypertension,diabetes mellitus,coronary heart disease,smoking,drinking,blood lipid,homocysteine(HCY),uric acid and posterior circulation vascular condition(lesion location,stenosis degree)were recorded.According to the final diagnosis,the patients were divided into two groups: posterior circulation ischemia group andnon-posterior circulation ischemia group.All patients were scored according to the modified ABCD2 scale,and their predictive ability was evaluated by using the receiver operating characteristic(ROC)curve.At the same time,Pearson chi-square test,t-test and rank sum test were used for single factor analysis,logistic regression analysis was used for multiple-factor analysis,and the related risk factors and prognostic factors of posterior circulation ischemia were counted.Results1.The modified ABCD2 score scale could provide valuable information for clinical diagnosis of isolated posterior circulation ischemia vertigo.When the modified ABCD2 score ? 4,the more likely it was to be diagnosed as isolated posterior circulation ischemia vertigo,with the sensitivity of 0.661,and the specificity of 0.627.2.There were significant differences in age,gender,history of hypertension,diabetes mellitus,coronary heart disease,stroke history,smoking,drinking and HCY between the two groups(P<0.05);there was no significant difference in blood lipid and uric acid between the two groups(P > 0.05).3.The results of single factor analysis showed that smoking,drinking,the lesion location and stenosis degree of posterior circulation vessels were the factors influencing the prognosis of patients with posterior circulation ischemia;the multivariate binary logistic regressionanalysis showed that smoking,drinking and moderate(50%-69%)and severe(70%-100%)stenosis of posterior circulation vessels were the independent risk factors affecting the prognosis.Conclusions1.The modified ABCD2 scale has a good diagnostic value for patients with isolated posterior circulation ischemia vertigo.When the score is ? 4,the more likely it is to be diagnosed as posterior circulation ischemia.2.Age,gender,history of hypertension,diabetes mellitus,coronary heart disease,stroke history,smoking,drinking and HCY are the risk factors of posterior circulation ischemia.3.Smoking,drinking and moderate(50%-69%)and severe(70%-100%)stenosis of posterior circulation vessels are independent prognostic factors of posterior circulation ischemia.
