| Objective(1)To assess the effect of the insulin interview in empowerment education for the type 2 diabetes mellitus on blood glucose and insulin injection behavior.(2)To assess the effect of insulin interview in empowerment education for the type 2 diabetes mellitus on the perceptions of insulinăempowerment ability and self-efficacy.MethodsThe study is a Quasi-experimental survey,selected 84 type 2 diabetes who were hospitalized in the department of endocrinology in a provincial hospital in Hunan province from May to August 2019.The May to June hospitalized 42 patients were set as the control group,July to August hospitalized 42 patients were set as the intervention group.The first questionnaire was completed within 24 hours after admission.The control group received routine health education.While the intervention group received routine nursing plus the insulin interview tool in empowerment education,conducted two rounds empowerment education during hospitalization.After discharge,the patients were followed up by telephone every two weeks to feedback their current insulin injection status and blood glucose level.The experimental group was given empowerment education model for their problems,while the control groupwas directly provided with information to answer their questions.Three months after discharge,a second questionnaire survey was conducted through telephone follow-up.Used chi-square test,t test and Wilcoxon W rank sum test to compare the scores of blood glucose,the perception of insulin,empowerment ability and self-efficacy of the two groups pre-test and 3 months post-test,as well as between the two groups pre-test and 3months post-test,repeated measures AVOVA was used to analyze the intervention effect.Results(1)General patients,information:84 patients were enrolled,5 were lost,and 79 were finally enrolled,including 40 in the intervention group and 39 in the control group.There were no statistically significant differences among gender,age,BMI,marital status,residence status,education level,per capita family income,medical expenses payment,time of disease,family history,complications,insulin use time,injection times,HbA1 c compliance rate between the two groups(P >0.05).(2)The assessment of the blood glucose:3 months after intervention,HbA1 c compliance rate and target blood glucose ratio in two groups were increased,and the increase was more significant in the intervention group(P < 0.05).(3)The assessment of the perception of insulin: After intervention,the total score of insulin perception in the intervention group decreased7.68 points,while that in the control group decreased 2.28 points.The improvement effect of insulin perception in the intervention group was better than that in the control group.(4)The assessment of the insulin injection behavior: After intervention,the insulin injection behavior score of the intervention group increased 9.30 points,while that of the control group increased 4.18 points.The improvement effect of the intervention group was better than that of the control group.(5)The assessment of the empowerment ability:The score of empowerment ability in the intervention group increased 0.34 points after the intervention,while that in the control group did not increase.The assessment of the self-efficacy: the difference of self-efficacy score in intervention effect,time effect and interaction effect was statistically significant(P < 0.05).The self-efficacy score of the intervention group increased 0.33 points,while that of the control group increased 0.15 points.According to the statistical results of interaction effect,the self-efficacy of the intervention group was better than that of the control group.Conclusions(1)The application of insulin interview tool in empowerment education can improve the HbA1 c compliance rate of T2 DM patients,reduce the proportion of hypoglycemia and hyperglycemia,and help patients control blood glucose well.The application of insulin interview tool in empowerment education can surmount T2 DM patientsâ cognitive barriers of insulin,establish the correct perception of insulin,and form the correct injection behavior.(2)The application of insulin interview tool in empowerment education can effectively improve the empowerment ability and self-efficacy of T2 DM,help the patients self-manage better and enhance confidence in coping with the disease. |
