Objective: With the improvement of people's living standards,malignant tumors seriously affect people's survival and health,and according to relevant reports,lung malignancies still account for the incidence and mortality of all tumors first,for the treatment of early lung malignancies,surgical treatment still dominates,minimally invasive,rapid rehabilitation medical concept of the development of medical concepts so that surgeons began to consider the same treatment effect in the case of less damage,thoracic surgeons began to explore whether partial excision can be applied to the treatment of early lung cancer The purpose of this paper is to explore whether there is any difference in the therapeutic efficacy and complications of anatomical pulmonary section excision and lymph node cleaning and pulmonary leaf excision and systematic lymph node cleaning in early lung cancer patients.Methods: In accordance with the 2019 NCCN guideline V6 phase,retrospective collection of early lung cancer in 2010-2015——2015 was surgically treated by thoracic mirror surgery,and the final routine pathology was confirmed as 156 casesof non-small cell lung cancer(before phase IB),divided into two groups according to surgical method,including 70 groups of pulmonary excision group,86 groups of pulmonary leaf excision group,statistical extraction time,intraoperative and postoperative and general incidence of hospitalization,average hospitalization time,The amount of hemorrhage,lymph node cleaning,no progression survival time,changes in lung function and five-year survival rate were processed with SPSS.25.0.data processing.Results: There were no significant differences in age,gender,preoperative pulmonary function,smoking,COPD,hypertension,diabetes,histological type,intraoperative blood loss,and lymph node dissection.Complications of pneumonia,atrial fibrillation,chylothorax,infection,and vocal cord paralysis were more common.Adverse reactions occurred in 3 cases(4.3%)in the lung segment group and 6 cases(8.1%)in the lung lobe group(8.1%).The operation time of segmental resection was significantly longer than that of thoracoscopic lobectomy(P = 0.002).From the perspective of lung function changes before and after surgery,the lung segment group(11)is smaller than the lung lobe group(19),with a significant difference(P <0.01).)Were smaller than the lung lobe group,with a significant difference(P <0.05).The survival time was measured by Keplan-Meier method.Seven patients(10%)had recurrence in the lung segment surgery group and 10 patients(12%)had recurrence in the lung lobe group.The five-year survival rate of the segment surgery group was 90%;the five-year survival rate of the lung lobe group was 88%;there was no significant difference between the two(P > 0.05)Conclusions: For early lung cancer patients,without increasing the risk,anatomical pulmonary segment excision has the same oncology effect as pulmonary leaf removal,and better retain lung function,whether it can replace lung leaf removal as the standard surgical formula of early lung cancer still needs to be confirmed by large samples.
