Objective:To explore the clinical effect of minimally invasive surgery(PVP,PKP)in the treatment of adjacent multi segment thoracolumbar vertebral compression fractures.Methods:From January 2013 to December 2016,206 cases,with adjacent multiple thoracolumbar osteoporotic compression fractures were collected in the departement of spinal surgery in Affiliated Hospital of Jining Medical College,and 192 cases were completely followed up,mainly in the case of two or three adjacent vertebral compression fracture,142 cases in two adjacent segments and 50 cases in three adjacent segments,a total of 434 vertebral bodies;two adjacent segments: 94 cases were divided into PVP group,a total of 188 vertebral bodies,15 cases were male,79 cases were female,the average age(69.63±1.57)years,48 cases were divided into PKP group,a total of 96 vertebral bodies,There were 10 male and 38 female,mean age(70.53±1.12)years;three adjacent segments: 34 cases were divided into PVP group,a total of 102 vertebral bodies,7 cases were male,27 were female,the average age(71.21 ± 1.34)years,16 cases were divided into PKP group,a total of 48 vertebral bodies.There were 4 male and 12 female,mean age(70.14±1.01)years;The VAS score(vision analogue scale),ODI(Oswestry disability index),sagittal vertebral anterior?posterior height difference((?))were collected before surgery,after surgery in one month,the end of follow-up,and generalinformation: BMI index(Body Mass Index,BMI),the average operation time,bleeding volume,bone cement injection volume,length of hospital stay were compared;The observed and the general clinical data were respectively analyzed in two and three vertebral osteoporotic compression fractures to find its clinical significance.Results:The subjects of this study were a total of 192 cases,which were completely followed up.The follow-up time were 12-24 months,average(12.03 ± 2.41)months;three segments and two segments: there was no significant difference in PVP and PKP groups in the general information difference(p>0.05);the two adjacent segments of OVCF:PVP and PKP group,before the operation?Postoperative in one month,VAS score,ODI index,there was no significant difference between groups(p>0.05);(?) showed no significant difference between the two groups in preoperative(p>0.05),in Postoperative one month and the end of the follow-up,The difference was statistically significant(p<0.05);the three adjacent segment OVCF: each observation index was no significant difference in the two groups before operation(p>0.05),in Postoperative one month and the last follow-up,it were statistically significant differences(p<0.05).In addition,the PVP group had 2cases of bone cement leakage.The patients had no obvious clinical manifestations.Conclusion:PVP or PKP,compared with conservative treatment,can quickly alleviate the symptoms of OVCF patients due to lumbar back pain,but for the adjacent multi segment OVCF patients,The long-term clinical effect ofPKP has a more obvious advantage than PVP.
