Pregnancy Outcome Of Patients With Fever After Spinal Canal Analgesia

Objective:Select the women who used intra-spinal tube analgesia during labor to study the relationship between intra-spinal tube analgesia and intrapartum fever;explore the delivery outcomes of pregnant women with postpartum fever after intra-spinal tube analgesia Midwifery,cesarean section)and neonatal outcomes(Apgar score,whether to transfer to pediatrics),etc.To explore the correlation between fever and poor pregnancy after intra-spinal delivery analgesia,and to explore possible preventive measures and treatment methods.Methods: Maternal and Child Health Hospital of Anhui Province was selected from November 1,2017 to October 31,2018 to all in-patients who voluntarily requested intra-canal delivery analgesia.100 patients with fever after spinal canal analgesia were randomly selected as the experimental group,and 100 patients without fever after spinal canal analgesia during the same period were selected as the control group.Changes in body temperature,first fever time,and fever after anesthesia were recorded.Cervical mouth expansion,fetal heart changes during fever,childbirth and fetal outcomes,prenatal and postnatal postpartum leukocytes,neutrophil percentage,C-reactive protein and procalcitonin,uterine secretion culture and other inflammatory indicators Pathological analysis and other indicators.Results: The body temperature of patients with spinal analgesia increased in 4 to 6 hours after anesthesia and recovered within 24 hours.The cesarean section rate was significantly increased in fever patients after spinal analgesia(60% vs 24%,P< 0.001).The cesarean section rate of patients with uterine cervix expansion less than 6 cm wassignificantly increased compared with that of cervix expansion more than 6 cm(P<0.05).The cesarean section rate was significantly increased in patients with a fetal heart rate exceeded 160 times per minute compared with those less than 160 times per minutes(P=0.044).Inflammatory indicators including white blood cell number,percentage of neutrophils,C-reactive protein,procalcitonin,and positive secretion of uterine secretions were significantly increased in fever patients(P<0.001),and decreased significantly within 48 hours after delivery.The incidence of amniotic fluid III pollution was significantly increased in the fever group compared with non-fever group(P<0.001).There were no significant difference of Apgar score and the NICU ratio of neonates between the fever group and the non-fever group.Conclusion: Postpartum fever caused by spinal canal analgesia mostly occurs 4-6 hours after anesthesia administration.When fetal fever occurs,some pregnant women experience a rapid increase in fetal heart rate and can not deliver cesarean section in a short time The rate increased significantly,but did not increase the incidence of puerperal infections or the incidence of adverse fetal outcomes.
