Objective: The aim of this study was to explore the effect of infarcted zone Tp-ec and Tp-e/QT on major adverse cardiac events(MACEs)in patients with ST segment elevation myocardial infarction for predicting the prognosis of STEMI patients and providing more valuable reference for clinical diagnosis and treatment.Methods: A retrospective analysis was performed on 281 cases of STEMI diagnosed in our hospital from January 2015 to June 2019.According to the occurrence or absence of MACEs,281 patients were divided into two groups and there were 36 cases in the MACEs group and 245 cases in the non-maces group.The clinical baseline data,laboratory biochemical parameters,and preoperative and postoperative Tp-ec and Tp-e/QT values of percutaneous coronary intervention(PCI)were compared between the two groups.The risk factors and predictors of MACEs in STEMI patients were screened by binary logistic regression analysis and receiver operating characteristic curve(ROC)analysis.Results: 1.Univariate analysis showed that: compared with non-MACEs group,MACEs group had higher age,faster heart rate,longer door to ballon time(D2B),higher proportion of women and multi vessel lesions,lower proportion of three grades of thrombolism in myocardial information(TIMI),lower left ventricular ejection fraction(LVEF),higher preoperative and postoperative Tp-ec and Tp-e /QT values(P<0.05).2.Univariate analysis showed that the preoperative and postoperative Tp-ec and Tp-e/QT values of MACEs group were higher than those of non-MACEs group(P<0.05).3.Multivariate binary logistic regression analysis showed that the preoperative Tp-e/QT(OR=1.238,95% CI 1.149-1.334,P<0.001),LVEF(OR=0.924,95% CI 0.874-0.976,P=0.005)and door-to-balloon time(OR=1.007,95% CI 1.001-1.012,P=0.018)were independent risk factors for MACEs in STEMI patients before PCI.3.The preoperative Tp-ec and Tp-e/QT under ROC curve(AUC)were 0.817(95%ci 0.763-0.864)and 0.889(95%ci0.843-0.926),respectively.Z test showed that the AUC of preoperative Tp-e/QT was significantly different from that AUC of preoperative Tp-ec,postoperative Tp-ec and postoperative Tp-e/QT(P<0.05).Conclusion: 1.The preoperative and postoperative tp-ec and tp-e /QT in infarcted none of the MACEs group were significantly increased,and the preoperative Tp-e /QT could effectively predict the occurrence of MACEs in the hospital.2.Tp-e/QT in preoperative infarct was an independent risk factor for MACEs in patients with STEMI.
