Intracranial hemorrhage(ICH)is one of the most common emergency cases in neurosurgery in China,and the main judgment index for rapid evaluation of ICH is the volume of intracranial hematoma.At present,the most commonly used calculation method in clinic is the multi-field formula method,whose principle is to mimic the intracranial hematoma into an oval shape,and calculate the hematoma volume according to the oval volume calculation method.However,most of the hematoma encountered in the clinic is irregular in shape and can not be fully formulated into oval shape,and the multi-field formula will no longer be applicable at this time.thus,we propose a 3 d-slicer calculation method based on 3 D modeling and compare the two calculation methods.Then we compare clinical application of 3 d-slicer guided puncture and traditional puncture drainage in irregular intracerebral hemorrhage,which provides the basis for the selection of treatment methods for patients with irregular supratentorial intracerebral hemorrhage.The second affiliated Hospital of Yan'an University and the first Hospital of Yulin City were selected for 60 patients with cerebral hemorrhage,and the selection time was December 2017 to December 2019.the hematoma morphology was classified by 5 classification method.Among them,rule group 25 cases,irregular group 35 cases.and then the hematoma volume was calculated by multi-field formula method and 3 d-slicer method,respectively.and then analyzed with statistical software.The second affiliated Hospital of Yan'an University and the first Hospital of Yulin City were selected for the diagnosis of cerebral hemorrhage,and the selection time was December 2017 to December 2019.A total of 109 cases were divided into conservative treatment group 55 cases,traditional puncture group 27 cases and 3 d-slicer group 27 cases according to the treatment method.statistical software was used to analyze the clinical data of each group.Results: In the first chapter,the volume of various morphological hematoma was compared,the three measurement methods were not significantly different in the application of regular hematoma(P >0.05),while the irregular hematoma was calculated by the software measurement method of multi-field formula and 3 d-slicer,and the results were significantly different(P =0.025).Analyzing the correlation of standard method,multifield formula method of irregular hematoma and software method of 3 d-slicer,it is found that when multifield formula method and 3 d-slicer are used to calculate,the volume of hematoma obtained is linearly related to the standard method.Chapter 2: Contrast 3 d-slic er group,conservative treatment group,traditional puncture group.The preoperative preparation time and operation time of 3 d-slicer groups were significantly higher than those of traditional puncture group(P<0.05).The time of intracranial hematoma absorption in conservative treatment group was significantly longer than that in puncture operation group(P<0.05),of which 3 d-slicer group took shorter time,and the incidence of various complications in puncture group was significantly less than that in conservative treatment group,especially the occurrence of pulmonary infection(P<0.05).The difference of hospitalization time and cost was statistically significant in the three groups(P<0.05),and in the d-slicer group(3)The length of stay and the cost of hospitalization were higher.Patients after treatment if the mRS?SSS is ?3,15 points,indicating good postoperative recovery,multi-factor Logistic regression analysis,complications,age,hematoma clearance time,hematoma classification and surgical methods are the risk factors for the prognosis of intracerebral hemorrhage hematoma resection,Conclusion 1.The multifield formula method can be used to estimate the volume of intracranial hematoma preliminarily,and its calculation of irregular hematoma volume is on the high side,while 3 d-slicer the software method to calculate irregular hematoma volume is more accurate and reliable,and the error of multifield formula method is avoided.According to the actual situation,we can judge the shape of hematoma and select the appropriate calculation method,so that we can calculate the volume of hematoma more accurately.2.The effect of 3 d-slicer puncture surgery for irregular supratentorial intracerebral hemorrhage is worthy of recognition,especially for primary hospitals,and its most important therapeutic significance may be to save it in the shortest time reversible nerve function injury,strive for maximum functional recovery,and improve the quality of life of patients.
