Objectives1.To evaluate the safety and efficacy of uterine artery embolization in the treatment of uterine fibroids2.To identify predictors associated with the clinical successMaterials and methodsA total of 83 women with uterine fibroids between 2011 and 2018 were retrospectively analyzed.At baseline and 6,12 months after operation,magnetic resonance imaging(MRI)was used to evaluate the volume of uterine and fibroids.The changes of clinical symptoms were recorded at baseline and 1,3,6 and 12 months after operation.Also complications and adverse events were recorded.ResultsThe success rate of bilateral uterine artery embolization was 98.8%(82/83).One case performed unilateral uterine artery embolization.97.6%(81/83)of the patients presented lower abdominal pain of different degrees,which could be completely relieved within one week(6.24±5.18 days).72.3%(60/83)patients presented vaginal secretion,which disappeared within 10.95±10.40 days.Two patients(2.4%)had severe infection and improved after anti infection treatment;11 patients(13.2%)presented amenorrhea;9 patients(10.8%)presented myoma expulsion;1 patient(1.2%)presented uterine rupture on MRI,which was considered to be related to previous myomectomy;3 case(3.6%)had normal pregnancy and delivery after operation.The volume of uterine and fibroids decreased by 37.4%and 63.5%(P<0.05)respectively in 12 months after operation.Dysmenorrhea relief was achieved in 88.6%(31/35),and the VAS score was 3.00±2.57 points lower than before.The symptoms of rectal and bladder compression were disappeared in 84.6%(11/13)and 88.9%(15/18)of the patients.The hemoglobin levels of 28 patients with anemia were increased postoperatively(p<0.05).The increase of menstrual volume before operation is an independent predictor associated with the reduction of myoma volume after operation.In patients with increased preoperative menstrual volume,the volume of myoma decreased significantly after operation.ConclusionUterine artery embolization is safe and effective in the treatment of uterine fibroids,the incidence of complications is low,postoperative pain can be relieved within a week?Age,location of fibroids and other indicators can not be used to evaluate the efficacy,the increase of preoperative volume can be used as an independent predictor of the clinical success.
