| Background:At the end of the 19th century,X-ray provides a good means of diagnosis and evaluation for fracture.But,as a result of high penetration degree in low density cartilage,X-ray can’t diagnose the fracture in children accurately.When the 21st century comes,the rapid development of musculoskeletal ultrasound,three-dimensional CT reconstruction and MRI makes the children fracture diagnosis more accurate.Children humerus lateral condyle fracture belongs to intra-articular fracture which needs high demand of reduction.The open reduction is an effective method,but in the actual clinical work,closed reduction and internal fixation can also bring satisfactory outcome.So it has become the preferred method for the treatment of humerus lateral condyle fracture in children.Objective:To explore the significance of ultrasonography in the diagnosis and evaluation of external condyle fracture of humerus in children,and to analyze the curative effect and advantages of closed reduction and internal fixation under the guidance of ultrasonic classification and arthrography.Methods:We analysis the data of 62 children diagnosed with lateral condylar fractures of the humerus by ultrasound from January 2016 to December 2019,then Calculate the sensitivity and specificity of fracture and displacement of cartilage diagnosed by ultrasonography,and compare the differences between closed reduction and open reduction in Song Ⅳ cases,such as operation time,fracture healing time,elbow function,loss of carry angle and complications.Results:1.The sensitivity and specificity of ultrasonic diagnosis of cartilage fracture of lateral condylar is 89.4%and 93.3%,the sensitivity and specificity of ultrasonic diagnosis of cartilage displacement is 82.4%and 100%.2.There was no significant difference between closed reduction and open reduction in fracture healing time,elbow function and carry angle loss when removing internal and external fixation,but the operation time was shorter,the elbow function score one month and two months after removing fixation was higher,and the lateral condyle spur and needle canal question were less in the closed group.Conclusion:1.Ultrasonography is an effective method for the diagnosis of cartilage fracture and displacement of the lateral condyle of humerus in children.2.Compared with open reduction,closed reduction and internal fixation in the treatment of Song IV cases has shorter operation time,faster recovery of elbow function and less complications. |
