ObjectiveTo explore the pathological mechanism and clinical treatment of cerebellar infarction through the clinical imaging changes of cerebellar infarction.Materials and MethodsThe clinical cases of cerebellar infarction were collected randomly.By observing the imaging findings and the outcome of clinical treatment,the imaging features of cerebellar infarction,the pathological mechanism of cerebellar infarction and the choice of treatment of cerebellar infarction were analyzed and discussed combining with relevant theoretical knowledge.ResultsAll cases of cerebellar infarction had obvious space occupying effect,edema and delayed enhancement starting from the cortex on MRI images.In one case,the condition was improved by dehydration-based treatment.In two cases,the condition did not improve after traditional conservative treatment,and even the focus and surrounding edema were enlarged.One case underwent surgery because of consideration for the tumor.The delayed enhancement of cerebellar infarction begins from the cortex because the capillaries in the infarcted area are still unobstructed,and the contrast medium can countercurrent into the capillaries through the vein and unobstructed capillary network.this phenomenon of venous backflow can improve the state of hypoxia in the infarcted area and provide pathological basis for the choice of treatment.Dehydration therapy can significantly improve the condition of patients with cerebellar infarction.ConclusionPatency of capillaries and venous reflux are important pathological mechanisms of cerebellar infarction,which change the pathophysiological state of infarcted area,prolong the duration of traditional ischemic penumbra,and make the treatment time window wider than cerebral infarction.The golden 6 hours of cerebral infarction is difficult to apply.Mannitol osmotic therapy combined with hormone is one of the first choice for clinical treatment of cerebellar infarction.Timely dehydration can even replace craniotomy decompression.
