| Background:Depression is a type of mood disorder caused by various reasons.Depression's main clinical feature is significant and lasting mental depression.The statistics report form WHO showed that the incidence of depression had increased by 18.4%from 2005 to 2015.Depression injures more than 300 million people and becomes a very huge economic burden worldwide today.On the other hand,insomnia is one of the most common symptoms of depression patients.The probability of depression and insomnia in one time is very high.More importantly,depression and insomnia can affect each other and aggravate the condition.At present,antidepressants represented by monoaminergic drugs have a slow onset of action and relatively high adverse reactions.In clinical,patients have poor adherence,are prone to residual symptoms after treatment.and have a relatively high recurrence rate,which places a great burden on individuals and society.Therefore,to study the neurological mechanisms of how depression and insomnia occur in common is very important.To develop new treatment of depression and insomnia except the monomines is very important.The significant association between depression and insomnia may have a unique neurological basis.In recent years,many scholars,all over the world,have made a lot of assumptions and researches on how depression and insomnia occur in a time and how they affect each other.Oxidative stress,immune and inflammatory responses,and circadian rhythm disorders are all pathologies related to depression and insomnia.Aiming at these pathways may improve the prognosis of patients with depression and insomnia,a large number of studies have shown that antioxidants can improve the symptoms of depression.Idebenone is an important component of ETC.Idebenone can promote ATP production and improve cell metabolism.Idebenone can effectively inhibit lipid peroxidation and scavenge oxygen free radicals,thereby protecting nerve cells,Improve the role of neurotransmitter metabolism.Objective:1.Review the research on the pathogenesis of depression and insomnia carried out in recent years all over the world,and analyze the possible neurobiological mechanism of depression and insomnia.2.Combine the application of Idebenone with antidepressants to treat patients with depression and insomnia,and observe the improvement of depressive symptoms and insomnia symptoms,and provide a reference for the treatment of depression and insomnia.Methods:206 patients with depression and insomnia were randomly divided into the control group and the observation group.After a comprehensive clinical diagnosis and assessment,the control group was given antidepressant therapy,and the observation group was given antidepressant and Idebenone(30mg/time,3 times a day)together for treatment.Symptoms in the two groups were assessed by the Hamilton Anxiety Scale(HAMA),the Hamilton Depression Scale(HAMD)and Pittsburgh sleep quality index(PSQI)before and 3 monthly treatment.Results:The HAMA,HMD,and PSQI scores of the control group and the observation group were significantly lower than those before the treatment.The treatment effect of the observation group was better than that of the control group,and the difference was statistically significant(P<0.05).The observation group could significantly improve the symptoms of depression,anxiety,sleep,and cognitive decline(P<0.05).Conclusion:A review of the literature found that inflammation and oxidative stress can play an important role in the pathogenesis of depression and insomnia by affecting the level of monoamine neurotransmitters,causing abnormal HPA axis glucocorticoid secretion,and causing abnormal activation of microglia.The study used idebenone as an antioxidant in combination with antidepressant drugs to treat patients with depression and insomnia,and conducted a controlled experiment with only antidepressant drugs,proving that idebenone combined with antidepressant drugs can more effectively improve depression symptoms,anxiety symptoms and insomnia symptoms. |
