Objective:Apply full automated measurement to assess coronary atherosclerotic heart disease(CHD)patients left ventricular systolic function,and explore the clinical value of automated measurement technology in the assessment of left ventricular systolic function in patients with CHD.Method:Establish an Access database containing the data of 500 subjects,127 CHD patients were retrieved meeting the inclusion criteria from the database as the case group,and were further divided into A group with abnormal wall motion and the B group with normal wall motion based on the left ventricular wall motion evaluation.127 non-CHD subjects meeting the inclusion criteria were randomly selected from the database as the control group.According to image quality score,three groups were further divided into the good quality subgroup("28-32score")andthe poor quality group("<28score")respectively.Using M-mode echocardiography Teichholz method(named "M-mode method"),two-dimensional echocardiography biplane Simpson method(named"biplane Simpson method"and the EchoPAC workstation Auto EF software(GE,version:110)(named "automated method")to measure left ventricular systolic function in each group.Observation indicators include left ventricular end-diastolic diameter and volume(EDV),end-systolic diameter and volume(ESV),ejection fraction(LVEF),stroke volume,fraction shorteningand measurement time.Evaluate the correlation and consistency of the left ventricular systolic function parameters measuredby different methods.Simultaneously,repeatability analysisofdifferent methods was performed.Result:The left ventricular systolic function in A group was lower than those in B group and the control group(all P<0.05).In all three groups,Correlations and consistency among left ventricular systolic function parameters measured by automated method and biplane Simpson's method were better than those measured by the M-mode method and the biplane Simpson method.The LVEF,EDV,and ESV measured by the automated method and the biplane Simpson method in all three groups achieved at least moderate correlations and good consistency.Even the LVEF measured by the two methods in the A group showed strong correlations(r:0.749,95%CI:0.516?0.871)and excellent consistency(ICC:0.809,95%CI,0.662?0.896).All differences were statistically significant(all P<0.05).The correlations and consistency of left ventricular systolic function parameters measured by different methods in image quality subgroups of each group were basically the same as those of the original groups.M-mode method and biplane Simpson method had good repeatability,while no differences were observed in automated method.The measurement time of the automated method was significantly shorter than that of biplane Simpson method(P<0.05),while it was similar with M-mode method(P>0.05).Conclusion:EchoPAC workstation Auto EF software(GE,version:110)is not only fast,simple,and easy to perform,but can also avoid the influence of human subjective factors,reduce the artificial measurement difference,and is less affected by the image quality.The level of correlation and consistency is relatively high,thus it has good applicability in both CHD patients and non-CHD populations,which even better in CHD patients with abnormal wall motion.EchoPAC workstation Auto EF software(GE,version:110)can provide a fast and accurate reference of left ventricular function evaluation for ultrasound doctors,especially for younger physicians with insufficient work experience,and contribute to the clinical treatment and prognosis assessment of patients.Figure[27]table[24]reference[130]...
