Objectives:?To explore the correlation between serum protein factor level and clinical symptoms and cognitive dysfunction of schizophrenia patients.?On the basis of this study, further explore the predictive effect of serum protein factor level and clinical symptom score on cognitive impairment degree of schizophrenia, and provide auxiliary methods for the treatment and prognosis of cognitive impairment in clinical schizophrenia patients.Methods:Seventy-eight patients with schizophrenia and 71 healthy controls were included.Serum protein factor concentrations(TNF-??BDNF?GFAP?S100??IL-6?NGF)of the patients and controls were detected by ELISA.Cognitive function was evaluated by MCCB cognitive test, and clinical symptoms of patients were evaluated by PANSS scale. To analyze the correlation between cognitive impairment, serum protein factor level and PANSS score in schizophrenic patients.Bayes discriminant function was established with serum protein factor concentration and PANSS total score as independent variables and MCCB cognitive domain defect degree as dependent variables to evaluate the severity of cognitive impairment in schizophrenia more objectively. Results:?The concentrations of Serum TNF-?[(62.00 25.02)pg/l vs.(32.3315.67)pg/1],S100?[(66.47 25.78)pg/1 vs.(46.22 19.62)pg/l],IL-6[(5.82 0.58)pg/1 vs.(4.44 1.39)pg/l]and NGF[(70.34 30.87)pg/1 vs.(53.41 9.40)pg/1]in the patient group were higher than those in the control group, while the concentration of BDNF in the patient group was lower than that of the control group[(2458.28 1146.60)pg/1 vs.(5209.35 438.48)pg/l],with statistically significant differences(P=0.000).?The patient group's cognitive factor information processing speed(SoP),word learning(VeL),reasoning and problem solving(RPS),visual learning(ViL)T scores were all lower than those of the control group, and the difference was statistically significant(p=0.000).?The total score of PANSS was positively correlated with TNF-?(r=0.390,p=0.000),S100?(r=0.291,P=0.010),IL-6(r=0.303,P=0.007)and negatively correlated with BDNF(r=-0.267,P=0.018)in the patient group.Positive symptom scores were positively correlated with TNF-?(r=0.305,P=0.007)and S100?(r=0.293,P=0.009).Negative symptom score was negatively correlated with BDNF(r=-0.492,p=0.000).?SoP T score in patient group was negatively correlated with TNF-?(r=-0.458,P=0.000),S100?(r=-0.414,P=0.000)?NGF(r=-0.302,P=0.007),and positively correlated with BDNF level(r=0.581,P=0.000);VeL T score was negatively correlated with(r=-0.352,P=0.002)?S1000?(r=--0.372,P=0.001),and positively correlated with BDNF level(r=0.619,P=0.000).RPS T score is negatively correlated with TNF-?(r=-0.255,P=0.024)?S100?(r=-0.255,P=0.024)?NGF(r=-0.323,P=0.004),and positively correlated with BDNF level(r=0.365,P=0.001).ViL T score is negatively correlated with TNF-?(r=-0.369,P=0.001)?GFAP(r=-0.253,P=0.026)?S100?(r=-0.329,P=0.003)?NGF(r=-0.363,P=0.001),and positively correlated with BDNF level(r=0.534,P=0.001).?The total score of PANSS in the patient group was negatively correlated with SoP(r=-0.494,P=0.000)?VeL(r=-0.460,P=0.000)?RPS(r=-0.328,P=0.003)?ViL(r=-0.359,P=0.001)T scores, the negative symptom score was negatively correlated with SoP(r=-0.287,P=-0.011)?VeL(r=-0.368,P=0.001)?ViL(r=-0.235,P=0.038)T points, while general psychopathology symptom score was negatively correlated with SoP(r=-0.287,P=0.011)?VeL(r=-0.278,P=0.014)T points.?The Bayes discriminant function model for the severity of cognitive impairment in schizophrenia was established by serum protein factor concentration and PANSS score. In the retrospective test, the accurate discriminant rates of four cognitive factors were SoP 82.9%,VeL 84.5%,RPS 77.6%,ViL 77.0%,and the accurate discriminant rates of cross test were SoP 71.1%,VeL 80.3%,RPS 76.3%,ViL 66.2%,respectively.Conclusions:The serum protein factor concentration, clinical symptom score and cognitive dysfunction score of schizophrenia are correlated.Bayes discriminant function model has a higher correct discrimination rate for the severity of cognitive impairment of schizophrenia. It is found that the level of schizophrenia-related protein factors and clinical symptom scores may have a predictive effect on the degree of cognitive impairment, which provides a more objective basis for the clinical efficacy evaluation of schizophrenia patients.
